| Objective: To investigate the correlation between coronary heart disease andposterior circulation infarction and its effect on prognosis. Methods: This study belongsto a retrospective case-control study, the research object for the2012March-2014yearin January in the patients with acute cerebral infarction treated in the Xinjiang UygurAutonomous Region people’s Hospital, requires cyclic group according to age, gender,body mass index, matched normal subjects served as normal control. According to infarctlocation will be in acute cerebral infarction patients were divided into the posteriorcirculation infarction group, anterior circulation infarct; according to the30day follow-upmRS score is divided into poor prognosis group (3-6) and good prognosis group (0-2group). Demographic data, vascular risk factors and imaging data, using logistic methodof single factor and multi factor regression analysis, the correlation between coronaryheart disease and cerebral infarction, evaluation of anterior circulation infarction andposterior circulation infarction, risk assessment, posterior circulation infarction beforeadverse prognostic factors. Results:371cases of acute cerebral infarction,343patientsmet the inclusion criteria, including anterior circulation infarction in204cases,139casesof posterior circulation infarction.139cases of normal controls. Anterior circulationinfarction combined and coronary heart disease in57cases (27.94%), posterior circulationinfarction combined and coronary heart disease in40cases (28.78%). The normal controlgroup of22cases (15.82%) in diagnosis of coronary heart disease.The results ofmultivariate analysis showed that, poster circulation infarction influence factor includingcoronary heart disease risk (OR2.052,95%CI1.316-3.198, P=0.002), hypertension (OR3.539,95%CI1.955-6.405, P=0.001), C reactive protein (OR1.064,95%CI1.009-1.112,P=0.023).Follow up of30days after posterior circulation infarction ocured, the mRSshowed a good prognosis92(66.18%), poor prognosis47(33.8%). Risk factors affectingthe posterior circulation infarction were coronary heart disease (OR0.225,95%CI0.08-0.65, P<0.01), admission NIHSS scores (OR8.702,95%CI3.51-21.58, P<0.01), GCS onadmission (OR3.433,95%CI1.05-12.44, P=0.03), pulse pressure (OR0.469,95%CI0.2- 0.95, P=0.037).Conclusion: The risk factors of poster circulation infarction werehypertension and coronary heart disease, C reactive protein. The main circulationinfluence prognosis factors of infarction with are coronary heart disease, pulse pressure,GCS score, NIHSS score. |
