| In China, cervical cancer ranks No. 9 of female cancer death. At present, the domestic and foreign scholars generally believe that type of high-risk human papilloma virus(HR-HPV) persistent infection is the necessary condition of cervical cancer and precancerous lesions occur but it is not the sole cause of the malignant cell transformation. Molecular pathology confirmed that playing an important role in the development of the occurrence of cervical cancer is the regulation of cell cycle disorder.p16 and p27 gene is the more important negative regulatory factor in Cyclin-CDK-CKI control system. It remains controversial that their expression in cervical epithelial cells and the study of the relationship with human papilloma virus. The experiment in order to detect the expression of p16 and p27 protein in the cervical exfoliated cells by flow cytometry, and explore its relationship with HR-HPV and cervical lesions, looking for a new auxiliary method for cervical cancer screening.We select 100 cases of cervical exfoliated cytology specimens from cervical cancer screening, screened from January 2013 to March 2014 at the First Affiliated Hospital of Hebei North University. One single positive group was followed up for at least 1 year HR-HPV continuous positive. All samples are confirmed by three experienced cell pathologist, and in strict accordance with the provisions of materials, all into the group of patients are informed choice agreement signed. This experiment adopts 2001 TBS and Cervista HPV HR diagnostic criteria. Double positive specimens were underwent colposcopic examination and biopsy routinely. We used flow cytometry detect the expression of p16 and p27 protein in each specimen.According to the result of the two-way screening, specimens can be divided into group of double negative, group of single positive, group TCT≤LSIL and HPV positive and group TCT≥HSIL and HPV positive. In combination with flow cytometry test results, we analyzed that the mean fluorescence intensity of p16 in each group of specimen were 58.08±2.63ã€60.12±3.32ã€61.48±3.48ã€68.06±7.09, and the mean fluorescence intensity of p16 in each group of specimen were 285.66±9.12ã€282.14±9.67ã€279.95±10.15ã€272.59±11.06. The expression of p16 in group TCT≥HSIL and HPV positive is higher than the rest of the group, the expression of p27 is lower than the rest of the group, the difference was statistically significant(P < 0.05). According to the results of colposcopy biopsy and flow cytometry detection, For 22 cases of inflammation and CIN1, 10 cases of CIN2 and 18 cases of CIN3 and CA group, mean fluorescence intensity of p16 in the order were 59.23±2.91ã€62.57±4.25ã€70.59±5.64, the p27 were 283.96±6.06ã€279.24±5.68ã€268.32±6.35. In the CIN3 and CA group, compared with the rest of the group, the expression of p16 and p27 obvious difference(P < 0.01). In group CIN3 and CA, group inflammation and CIN1, HR-HPV of group A9 were 13/18(72.22%), 5/22(22.73%), the difference was statistically significant(P < 0.05). In single positive group, the rate of group A9, group A5/6, group A7 and group of mixed infection were(43.33%), 8/30, 13/30(26.67%), 5/30(16.67%), 4/30(13.33%), thus, the infection rate of group A9 was obviously higher than that of other groups.In summary, with the progression of cervical leisions, p16 expression was increased, p27 expression decresased, and both infection associated with HR HPV. The expression of p16 and p27 in LSIL and below with HSIL and above group was statistically difference(P < 0.05), which can realize their split, improve the targeted biopsy. |
