Objective: To study the effectiveness and safety of the spinal canal decompression combined with semi rigid fixation system(Waveflex) with internal fixation and Spinal canal decompression and interbody fusion and pedicle screw fixation system for the treatment of lumbar degenerative disease, to provide for the clinical treatment of lumbar degenerative disease provides a new idea and solution.At the same time, to observe the effect of two different operation on the activity of patients lumbar.Evaluation of semi rigid fixation system for the prevention of spinal fusion of adjacent segment degeneration(ASD) effect.Methods: This study adopts the method of block parallel control and Before and after operation control,Statistical analysis of 2013 September to 2014 September in treatment of patients with spinal operation in the Department of orthopedics, Affiliated Hospital of Shandong University of Traditional Chinese Medicine, including 76 patients with single segmental lumbar degenerative disease, 45 cases with lumbar spinal canal stenosis, 15 cases of lumbar spondylolisthesis, lumbar disc herniation and spinal stenosis in 16 cases.According to the operation mode, divided into the spinal canal decompression combined with semi rigid system(Waveflex) and internal fixation group, group B: 35 cases, spinal canal decompression and interbody fusion and pedicle screw system fixation group, group A: 41 cases.Patients were followed up for 3 to 12 months, average 6 months, with the last follow-up data as a basis for evaluation.By using the visual analogue scale(VAS) and Oswestry disability index(ODI) to evaluation of the symptom severity before operation and improvisation after operation of the two groups patients.Imaging measurement of displacement and power for on-chip angle, to measure the lumbar flexion and extension function(L2-S1)(ROM1) and semi rigid fixed segment activity(ROM2).In addition, operation time, intraoperative blood loss, postoperative hospital stay, complication of operation, recovery time also as observation index.Follow up of patients, evaluation of patients with VAS and ODI score, and lateral lumbar x-ray with hyperextension and hyperflexion check.Results: through spss20.0 statistical analysis, using t test analysis,A group of patients with operation time, intraoperative blood loss was higher than that in group B(P<0.05),All patients with VAS score and Oswestry disability index(ODI) was significantly better than that before operation(P<0.05),Two groups of patients in VAS and ODI scores had no significant difference on the rate of improvement,No statistical significance in patients of group B before and during the whole follow-up operation of lumbar and segmental mobility differences,Group A patients of lumbar spinal motion loss greater than the patients in group B(P<0.05).Conclusion: semi rigid fixation system(Waveflex) internal fixation compared with traditional bone grafting fusion and screw rod system fixation with shorter operation time, less bleeding and other advantages in operation.And can effectively reduce the activities of operation lumbar loss,reducing the occurrence of adjacent segment degeneration after spinal fusion(ASD) and there is no significant difference in the efficacy of two kinds of operation mode.Therefore, a clear diagnosis, operation indications clearly, choose a reasonable way for the treatment of lumbar degenerative diseases can achieve satisfactory results.
