Objective: based on the theory of "action",to explore Waveflex hybrid internal fixation system in the treatment of multiple segmental lumbar degenerative disease of the clinical curative effect,based on strong internal fixation,Waveflex two hybrid internal fixation operation method of comparative study between groups and the Waveflex hybrid preoperative postoperative fixation system group contrast research,assessment Waveflex hybrid effect of internal fixation system operation,provide the basis for further clinical application.Methods: from August 2016 to March 2018,the medical records of 60 patients diagnosed in the department of spine orthopaedics of the affiliated hospital of shandong university of traditional Chinese medicine who met the diagnosis of lumbar degenerative diseases and underwent surgical treatment were collected.The 60 patients were divided into hybrid internal fixation group(30 cases with Waveflex hybrid internal fixation system)and rigid internal fixation group(30 cases with traditional rigid internal fixation system).The subjective feelings(VAS,ODI)and objective evaluation indexes(operation time and blood loss)of the two groups of patients before,after and at the last follow-up were statistically analyzed to evaluate the effectiveness and safety of the Waveflex hybrid internal fixation system.Statistical analysis was performed on the imaging data(LA,DSH,HIF,ROM),Pfirrman grading of intervertebral disc,and the imaging grading of intervertebral space degeneration of patients before and after the application of the Waveflex hybrid internal fixation system.The stability of the system and its influence on adjacent segments and secondary responsibility stages were evaluated.Results:Sixty patients were followed for 9-25 months,an average of 16 months.(1)there were statistically significant differences between the hybrid internal fixation group and the traditional internal fixation group in operative time and intraoperative blood loss P<0.01.(2)in the hybrid internal fixation group,VAS score 2 months after surgery and at the last follow-up was lower than that before surgery,P<0.05 was statistically significant.There was no significant difference in VAS score P>0.05 between the hybrid internal fixation group and the traditional internal fixation group 2 months after surgery and at the last follow-up.(3)the ODI score in the final follow-up of hybrid internal fixation group was lower than that before the operation,and P<0.05 was statistically significant.There was no significant difference in the final follow-up ODI score P>0.05 between the hybrid internal fixation group and the traditional internal fixation group.(4)in the hybrid internal fixation group,the results of LA,DSH and HIF 2 days after surgery and the final follow-up were better than those before surgery,with a statistically significant difference of P<0.01.There was no statistically significant difference of P>0.05 between the final follow-up and the two days after surgery.(5)in the hybrid internal fixation group,there was no significant difference in lumbar spine activity(ROM1)two months after the operation and at the last follow-up visit compared with that before the operation,and no statistical difference was found in P>0.05.In the hybrid internal fixation group,the dynamic fixation segment lumbar spine activity(ROM2)decreased two months after surgery and at the last follow-up as compared with that before surgery,P<0.05 was statistically significant.30 cases(6)this study applied Waveflex hybrid internal fixation system treatment in patients with multiple segmental lumbar degenerative disease Waveflex segmental lumbar Pfirrman classification of grade 12 cases by Ⅴ Ⅳ,7 cases by levels Ⅳ into Ⅲ,4 cases byⅢ level into Ⅱ level.(7)according to the imaging of intervertebral disc degeneration,UCLA class 30 patients had 12 cases by Ⅱ Ⅰ level.Conclusion: Waveflex hybrid internal fixation system has the advantages of short operative time and less intraoperative blood loss compared with traditional rigid internal fixation system in improving the clinical symptoms of patients.At the same time,the Waveflex hybrid internal fixation system can maintain the stability of the lumbar spine,control the abnormal activity between vertebral bodies,retain the segmental activity function,delay the degeneration of adjacent segments,and create good conditions for the recovery of lumbar intervertebral disc. |
