Objective: By studying the platelet levels change of the patients with C irrhosis of the liver in decompensation period, analysising its related factors, exploring the role and significance of the platelet in cirrhosis of the liver in decompensation period.Method: Summmary189 cases of patients with hepatitis cirrhosis of the liver(including hepatitis B cirrhosis and hepatitis C cirrhosis of the liver) in decompensated period, 75 patients with alcoholic cirrhosis of the liver in decompensation period, 59 patients with autoimmune cirrhosis of the liver in decompensation period, all have been diagnosed with C irrhosis of the liver, from June 2013 to December 2014 in i digestive department of the second affiliated hospital of Dalian medical university. And select 50 physical examinations as a control. Recording each patient’s basic information, signs, common complications(primary liver cancer, gastrointestinal bleeding, hepatic encephalopathy, cholelith disease, infection, electrolyte disorder) and laboratory examination(platelet count, platelet distribution width, mean platelet volume, big ratio of platelet, white blood cells, neutrophils absolute value, red blood cells, hemoglobin value, mean corpuscular volume, red blood cell volune distribution width, alanine aminotransferase, aspartate aminotransferase,AST/ALT, γ- glutamyl transferase, alkaline phosphatase, total bilirubin, albumin, prealbumin, blood glucose, cholesterol, triglycerides, creatinine values, serum sodium, Serum potassium, prothrombin time, prothrombin time, international normalized ratio, activated partial thromboplastin time, thickness of spleen, portal venous trunk diameter, spleen vein diameter).Analysis related factors of the change of the platelet levels.SPSS22.0 statistical software for statistical analysis:the t test is used to compare the differences between two different groups of measuring data;the single factor analysis of variance is used to compare the differences betweenmultiple sets of measurement data; RⅹC c2 is used to compare the counting data;P < 0.05 means the difference is statistically significant.Result: 1.Platelet levels in patients with cirrhosis were lower than normal, PDW is higher than normal;2.The platelet level in patients with cirrhosis accompanied by splenomegaly, hepatic encephalopathy, cholelith disease is lower than that of the patients corresponding non-splenomegaly, no hepatic encephalopathy, no cholelithiasis;3.The platelet levels of the patients with primary liver cancer, electrolyte disorder is higher than that of patients without primary cancer and electrolyte disorder, the difference was statistically significant(P < 0.05);4.Platelet is positively correlated with white blood cells, neutrophils, red blood cells, hemoglobin, albumin, prealbumin, cholesterol, triglycerides, creatinine, Serum potassium, and is negativeiy correlated with mean corpuscular volume, platelet distribution width, alanine aminotransferase, aspartate aminotransferase,AST/ALT, total bilirubin,, blood glucose, prothrombin time, prothrombin time, international normalized ratio, activated partial thromboplastin time, thickness of spleen. Conclusion: 1. Liver cirrhosis can lead to platelet levels drop;2.The splenomegaly and spleen hyperfunction is not the only reason that making platelet levels down, alcohol, viruses, autoimmune factors, tumor cells, infection, electrolyte disorder etc all can affect platelet levels;3. The platelets can be used as evaluation of the complications and the prognosis in patients with cirrhosis.
