Clinical Study Of Treating Primary Ovarian Insufficiency With Bushenhuoxue Recipe Combining With The Recognition Of TCM Constitution

Objective:To observe the intervention effect of the therapy of Bushen Huoxue Decoction on the primary ovarian insufficiency(POI) caused by various factors.To investigate the TCM constitution type distribution characteristics of the patients, to provide the basis of early diagnosis and prophylactic treatment.Methods: 43 patients who met the diagnostic criteria for POI were divided into two groups: therapy group (22 cases, treated with Bushen Huoxue Decoction) and controlled-group of (21 cases, artificial cyclic therapy:progynova and progesterone capsule sequential therapy for 3 cycles). The constitutional type characteristics were observed between the POI-group and the normal-group.The changes of clinical symptoms,the menstrual cycle,serurn hormone(E2, FSH, LH), ovary blood flow indices(OVD、AFC、Rl)were observed before and after treatment. And the differences between therapy-group and controled-group were observed.Results:1. In terms of serum sex hormones, in both therapy-group and controled-group,the level of E2 was increased (P<0.05), the level of FSH and LH had no significant difference between after and before treatment (P>0.05).2. In terms of B ultrasonic, in both therapy-group and controled-group, the AFC was increased (P<0.05).The RI was decreased in therapy-group(P<0.05),the controlled-group has no significant difference. In both groups,there were no significant difference in OVD.3. At the effect of comprehensive therapeutic effect,the total effective rate of the therapy-group was 91%,the controlled-group was 81%;the TCM symptoms effective rate of the therapy-group was 86%,the controlled-group was 76%;the menstrual cycle and clinical symptoms were improved in the two groups, and the improvement of menstrual cycle in controlled-group was better (P<0.05), but the symptoms of waist and knee sour, vaginal dryness, insormnia and dreaminess were obviously better than controlled-group (P<0.05).4. Three months after the therapy, the menstruation symptoms score has no significant difference compared to three months ago (P>0.05) in therapy-group, in controlled-group there was no significant difference (P>0.05) between three months after the therapy and before treatment, there was a significant difference (P<0.05) between three months after the therapy and after treatment.5. In terms of physical observation,in POI pantients biased constitution was more; the phlegm dampness stagnation were higher than others,followed by Yin deficiency,blood stasis and Yang deficiency. In normal control group,the majority with normal constitution,Yang deficiency and phlegm dampness. There was significant difference between the two groups(P< 0.05).Conclusion:1.Both Chinese medicine and Western medicine can increase E2 level, regulate the reproductive endocrine axis, improve the ovarian blood supply, which can improve the function of ovary.2.Bushen Huoxue Decoction can obviously improve the ovarian dysfunction in patients with clinical symptoms, especially in improving the symptoms of waist and knee sour, vaginal dryness,insormnia and dreaminess is superior to controlled-group;during the treatment the improvement of the irregular menstruation was obvious in controlled-group, but after three months the symptoms of irregular menstruation repeatedly, and there was no significant difference in therapy-group.3.
