| Objective Preliminary discussion the application value about iflow time mappingmeasurement of (Lt-Kt)ã€(Ka-La) between the liver and kidney,real-time tissue elastography (RTE) in patients with chronic hepatitis B liver fibrosis, in order to conclude with the evaluation of liver fibrosis stage is closely related to the parameters.Method Collect the number of 47 patients with chronic hepatitis B with liver biopsy, all the subjects for regular ultrasound, iflow time mapping and real-time tissue elastography before liver biopsy, real-time tissue elastography measurement index of liver fibrosis (LF), iflow time mapping measurement of Lt-Ktã€Ka-La. In pathology, the degree of fibrosis stage as the gold standard, divided into SO-S45 groups, the Lt-Ktã€Ka-La and LF, correlation analysis of liver fibrosis, SO and S1, S2, S3, S4 for different research group for the receiver-operating characteristic curve (ROC) analysis, and the area under the curve (AUC) of the three.Results 5 groups of different levels of liver fibrosis patients in ultrasonic quantitative compare the difference of each index, the patients have no differences about statistical significance in age and gender, but in Ka-La, the organization dispersion quantitative differences of quantitative indicators was statistically significant (P< 0.001), the Lt-Kt difference indexes statistically significant differences between parts. Chronic hepatitis B patients with fibrosis degree aggravating, The Lt-Kt increases gradually, the diagnostic reference for S0:4.17± 1.8s, S1:6.49± 1.8s, S2,8.63± 1.53, S3:11.2±2.9s, S4:15.56±2.65s. The diagnosis accuracy is respectively:71.4%ã€83.3%ã€84.6%ã€87.5%ã€91.7%.With the fibrosis degree aggravating, the Ka-La increased gradually, the diagnostic reference for S0:2.59±0.9°, S1:4.14± 1.55°, S2:6.33± 1.3°, S3:9.38± 1.67°, S4:14.83± 1.53°. The diagnosis accuracy is respectively:80%ã€83.3%ã€86.7%ã€87.9%ã€91.7%. Also along with the LF increase gradually, the reference threshold for S0:1.78±0.35, S1: 2.24±0.45, S2:2.77±0.29, S3:3.25±0.59, S4:3.80±0.21, diagnostic accuracy is respectively:85.7%ã€87.5%ã€90.9%ã€91.2%,91%.Conclusion Iflow time mapping and real-time tissue elastography of two kinds of technology assessment of liver fibrosis degree, high sensitivity, reliable method and measured value is stable, strong repeatability, but iflow time mappingoperation process is tedious, time-consuming, and require the operator and the height of the client to cooperate, these reasons make the ultrasonic imaging of lesion degree of no RTE accurate judgment,iflow time mappingis not convenient for clinical application. |
