| Objective : To explore Traditional Chinese Medicine(TCM) syndrome elements distribution of psoriasis vulgaris(PV) as well as some association between lesions and TCM syndrome elements, with the expect to provide reference for clinical diagnosis and treatment.Methods: I reviewed literatures on causes and mechanism of PV and summarized research progress on causes and mechanism of PV of both modern medicine and TCM. Based on this, the clinical questionnaire was designed and 173 cases of PV were included for their general information, disease progression as well as symptoms and signs etc. All the data were input into Excel to make a symptom database. Syndrome elements were got with syndrome differentiation and through statistic analysis and data mining the PV syndrome elements distribution and the association between syndrome elements and lesions were explored.Results: PV syndrome elements distribution ranking from high to low was qi deficiency, Yang deficiency, qi depression, blood stasis, dampness-heat, coldness-dampness, blood-heat, qi stagnation, blood deficiency, fluid insufficiency and Yin deficiency. Through association rules, the association found that met the mini-support and the confidence ranked high included skin color →Yang deficiency, pale red →Yang deficiency, dark red →blood stasis, coin size →Yang deficiency, map shape →Yang deficiency, general lesions →blood stasis, head-distributed →qi deficiency, back-distributed →qi depression, chest-distributed →blood heat, lower limbs-distributed →qi deficiency, not itching →Yang deficiency and slight itching →qi depression etc.Conclusion: The mechanism of PV involved coldness, heat, deficiency and excess but deficiency and intermingled deficiency and excess were more common. In addition, the relative fluids insufficiency affected more universally than the absolute. The lesions of skin color and pale red suggests that the potential mechanism be Yang deficiency, and dark red blood stasis; coin size with drop size Yang deficiency and blood stasis, coin size and map size Yang deficiency; general distribution, head-distributed and lower limb-distributed deficiency, back-distributed and chest-distributed excess; no-itching Yang deficiency and slight itching qi depression and Yang deficiency. PV is characterized as long-lasting process and complex mechanism and clinical treatment shall be based on comprehensive syndrome differentiation which can effectively guide treatment and improve effects. |
