The Clinical Observation Of Qiliqiangxin Capsule In The Treatment Of Chronic Heart Failure Between Tcm Syndrome Type

Object: This study was under the guidance of syndrome differentiation and treatment of Chinese medicine, using Western medicine combined with Qiliqiangxin capsulefor treating chronic heart failure, by observing the changes of the life quality, cardiac function,BNP in the whole blood, LVEF(left ventricular ejection fraction index) and other indicators in the different TCM syndromes of chronic heart failure patients after the Integrative Medicine,and analysing the data dependency,to evaluated the therapeutic effects in different syndromes of chronic heart failure with traditional Western medicine combined Qiliqiangxin capsule,to revealed its mechanism,for providing new scientific basis in the treatment of chronic heart failure with Integrative Medicine,and for widening our thinking of the diagnosis and treatment of chronic heart failure and the prevention research.Methods:Selecting 53 cases with chronic heart failure in 2013 Oct.-2014 Dec.in line with the diagnostic criteria for heart failure(Framingham heart failure diagnostic criteria, 2011《24 Professional 105 diseases TCM treatment program》on the chronic stable heart failure symptom sub-type) in Hubei Province ChineseHospital Cardiology wards and outpatient patients,and divided into five groups(heart-lung qi deficiency group, qi and yin deficiency group, yang deficiency group, the kidney essence loss group and the control group).According to the disease treated with conventional Western medicine treatment of heart failure,including bed rest, oxygen,angiotensin-converting enzyme inhibitors(ACEI)or angiotensin II receptor blockers(ARB),β-blockers,aldosterone antagonists,vasodilators(nitrates,sodium nitroprusside,etc.),with or without digitalis,diuretics and statin drugs,etc.In addition to the control group, on the basis of the conventional treatment of heart failure,oral the Qiliqiangxin capsules(from Huangqi,Fuzi, ginseng,Tinglizi,Dansen and other components),every 4,3 times a day for 12 weeks.Were observed the quality of life(using the Minnesota Heart Failure life quality Questionnaire Chinese version), whole blood NT-pro BNP,echocardiography in LVEF before treatment and after treatment.Using SPSS17.0 statistical analysis software package for analysis.Research: After statistical analysis,the five groups of patients were not statistically significant(P>0.05)in terms of gender, age,primary disease spectrum,cardiac function,the incidence of the course and so on.Compared with pre-treatment,the five groups of patients Minnesota quality of life score was significantly decreased after 3 months treatment,after treatment with a statistically significant difference between groups(P<0.05),and combination therapy groups compared with pre-treatment P<0.01,there is a more significant difference compared with the control group. Five groups after 3 months treatment were improved cardiac function compared with pre-treatment,and the total efficiency of each groupin Integrative Medicine is better than the control group, especially yang deficiency group was much better than the other groups. The NT-pro BNP levels of every group was declined after treatment compared with pre-treatment,and combination therapy groups compared with pre-treatment P<0.01,is more statistically significant compared with the control group. Compared with pre-treatment, after treatment yang deficiency group and the kidney essence loss group P<0.05,were statistically different.After treatment, each group left ventricular ejection fraction was significantly increased, there were statistically significant(P<0.01)compared with pretreatment, and combination therapy groups compared to the control group to improve the LVEF is more obvious. After treatment, heartlung Qi deficiency group, yang deficiency group and the kidney essence loss group were compared with control group P<0.05,and yang deficiency group compared with the kidney essence loss group P<0.05,with a significant difference. All patients included in this study were successfully completed clinical observation of three months, patients in each group were did not appear serious adverse events, and five groups safety parameters were not detected abnormal changes during treatment and after treatment.Conclusion: Qiliqiangxin capsule in the treatment of chronic heart failure syndrome type could significant improvement the clinical symptoms, improve the quality of life of patients, and reduce blood NT-pro BNP levels, and improve left ventricular ejection fraction, with conventional treatment with western medicine, tackling the problem, so that the overall coordination of Chinese medicine could combined with symptom control of Western medicine, and achieved good results in clinical efficacy, especially in the patients withyang deficiency group the efficacy is more significantly, is safely and effectively treatment of chronic heart failure in the compound Chinese medicine preparations.
