| Objective: Through the establishment of quantified diagnosis model of deficiency of both yin and yang of kidney syndrome of hypertension, we tried to realize the standardization and objectivity of deficiency of both yin and yang of kidney syndrome of hypertension.Methods: The information of four methods of diagnosis was based on ancient and modern literature review, and Delphy of expert questionnaire was applied to form the first edition of scale. Discrete trends method, systematical clustering method, the Krone Bach coefficient method, analysis of distinction, the correlation coefficient method, factor analysis were applied to analyze the data of clinical epidemiology for item selection. If the item was deleted in one of the statistical method, resultly, it was deleted in the second edition of scale. Subsequently, the second edition of scale was tested for practicability, inter-scorers reliability(including retest reliability, split half reliability, homogeny reliability, scorer reliability, Intra class correlation coefficient), validation(surface validity, discrimination validity, construct validity) and degree of reaction. With another clinical epidemiology of the second edition of scale, we established subjective, objective and integrated weight coefficient, and threshold value of diagnosis. Lastly, the scale was tested for its clinical application value by retrospective and prospective test.Results: This research established syndrome of deficiency of both yin and yang of kidney of hypertension diagnostic scale. Compositive tests showed the diagnostic scale possessed good practicability, inter-scorers reliability, validity and degree of reaction. We figured out the diagnostic threshold value was 255.0. Clinical diagnostic test showed, this scale possessed good sensitivity, specificity, correct diagnosis index, accuracy, high positive likelihood ratio and low negative likelihood ratio.Conclusion: Based on literature review, Delphy of expert questionnaire, clinical epidemiology research and multiple statistical analysis, the syndrome of deficiency of both yin and yang of kidney of hypertension diagnostic scale had good practicability, accuracy and clinical application value. The present findings would provide objective and standard scale for further research, and thinging and methods for the standard research of the combination of disease and syndrome. |
