Objective Neurogenic bladder after spinal cord injury is due to a bladder dysfunction of micturition reflex control to appear in central nervous system after spinal cord injury. The characteristics of neurogenie bladder caused by spinal cordin jury are high incidence, more deaths, long treatment time,etc. Seriously affect the daily life and the life quality of patients.There are many treatments about the disease. But the shortages of the treatments are working slowly, high cost, curative effect of difficulty determine in a certain extent. So seeking effective treatments for NB after SCI and let patients back to family and society are the problem of medical profession need to continuously explore and research.This subject adopts the electric acupuncture combined with magnetic stimulation of sacral roots for NB after SCI.On the basis of clinical observation, by comparing the voiding diary and urine flow dynamic change, evaluate the quality of life score which affect by the urination thus clear improvement of bladder dysfunction of SCI-NB after getting the treatment of electric acupuncture combined with magnetic stimulation of sacral roots mainly,further to explore the feasibility and clinical advantages, summarizes a.method which is distinct curative effect, simple and suitable for clinical practice.Methods 80 patients with neurogenic bladder patients following spinal cord injury all were treated with drinking project and intermittent catheterization.And they were randomly divided into four groups, each group consisting of 20 patients. Group A was a general group which oral neurotrophic drugs. Group B was an electro-acupuncture group which treated in the general group in the foundation, provided electro-acupuncture at relevant acupoints. Group C was a magnetic stimulation group which treated in the general group in the foundation, provided magnetic stimulation at the sacral nerve roots. Group D was a combination group which combined with above three groups of treatment. Each group spent 4 weeks undergoing appropriate treatment.Urinate diary,urodynamic investigation and quality of life score were applied before and after treatment.Results After this study, a total of 80 patients observed SCI NB, shedding one cases of group A, group D shedding one case, and finally completed the study of 78 patients.1. Comparison of the general clinical data in four groups before treatment: The general clinical data in four groups before treatment(including age, sex, ASIA classification, injury course) had no significant difference(P>0.05), with homogeneous with comparable.2.The change of urinate diary and urodynamic investigation in four groups before and after treatment: Each evaluate quota of urinate diary and urodynamic investigation had no significant difference among four groups before treatment(P>0.05).In four groups, after treatment,each evaluate quota of urinate diary and urodynamic investigation was higher than before treatment(P<0.05).But each evaluate quota in group Bã€group Cã€group D was higher than that in A group after treatment(P<0.05). There is no significant difference of each evaluate quota between group B and group C(P>0.05). Group D of evaluation quotas were better than that of group B and group C(P<0.05).3. The quality of life score which were influenced by voiding symptoms in four groups before and after treatment: Compaired with pre-post treatment, the quality of life score had statistical difference(P < 0.05). The score of group D significantly lower than group A, group B and group C(P<0.05). There is no significant difference of the score between group B and group C after treatment(P>0.05).Group B and group C was lower than that in group A(P<0.05).Conclusion These findings suggest that electro-acupuncture 〠magnetic stimulation of the sacral nerve roots and electro-acupuncture combined with magnetic stimulation of the sacral nerve roots mainly on the NB after SCI in patients are effective. They can improve the average 24 h emiction frequency, micturition of the 2 time intervals and urine volume. We can find the combination group was significantly better than the electro-acupuncture group and magnetic stimulation group. To improve voiding dysfunction, electro-acupuncture combined with magnetic stimulation of the sacral nerve roots as main therapy is better than single treatment method.It is a comprehensive and effective treatment,worthy to be populared and applied in clinics. |
