Objective Observation of insulin-like growth factor-1(IGF-1) levels in the serum of varying severity in diabetic nephropathy(DN) patients, and to explore and further analysis of blood IGF-1 and DN severity of relevance, provide basic theory for early prevention of diabetic nephropathy in accordance with.Methods According to 1999 WHO diabetes diagnosis and classification criteria selected Affiliated Hospital of Hebei United University, Department of Endocrinology, March 2013~T2DM patients hospitalized in March 2014 of 120 cases. According to urinary albumin excretion rate(UAER) in patients with type 2 diabetes were divided into four categories:non-nephropathy in type 2 diabetic group(DM), UAER<20ug/min; early diabetic nephropathy(DN1 group), UAER 20~200ug/min; clinical diabetic nephropathy group(DN2 group), UAER>200ug/min, 24 h urinary protein excretion>0.5g, normal renal function; end-stage diabetic nephropathy group(DN3 group) UAER>200ug/min, 24 h urine proteinuria>0.5g, abnormal renal function. All subjects were excluded acute complications of diabetes, hypertension kidney damage, glomerulonephritis and pyelonephritis, urinary tract infection, kidney cancer and other diseases. Affiliated Hospital of Hebei United University, the same period 30 cases of examination as a control group of normal healthy people. All subjects than 8h fasting, urine ablumin in urine urine specimens,morning venous blood 5ml detection of insulin-like growth factor-1(IGF-1), glycated hemoglobin(Hb A1c), fasting plasma glucose(FPG), growth hormone(GH), serum total cholesterol(TC), triglyceride(TG), Low-density cholesterol(LDL-C), high density lipoprotein cholesterol(HDL-C)and glomerular filtration rate(GFR) and other indicators.According creatinine levels combined age, weight, sex according to a formula glomerular filtration rate(GFR). All data analysis applications IBM SPSS 19.0 statistical software package for statistical analysis, for the measurement data using mean±standard deviation.ANOVA was used to compare between the two groups were compared using the t test,pairwise comparisons among groups using LSD test. Multivariate statistical correlation between the use of logisitc regression analysis, linear correlation analysis multiple regression analysis. Test level, P<0.05 was considered statistically significant.Results 1 Type 2 diabetes population in each group, HbA1 c, FPG, TC, TG, SBP levels were higher, HDL-C levels lower than the control group.Diabetic nephropathy groups Hb A1 c, FPG, TC, TG, SBP levels were higher than non-diabetic nephropathy, HDL-C levels lower than non-diabetic nephropathy. The difference was statistically significant(P<0.05). Diabetic nephropathy in type 2 non-glomerular filtration rate is slightly higher in type 2 diabetic nephropathy glomerular filtration rate decreased end-stage diabeticnephropathy glomerular filtration rate to a minimum. Description of type 2 diabetes in the non-diabetic glomerular filtration rate increased slightly, with the degree of renal impairment increased glomerular filtration rate gradually decreased. Clinical diabetic nephropathy, patients with end-stage renal disease group longer duration of diabetes type 2diabetic nephropathy group of non-early diabetic nephropathy group, indicating that the longer the duration of diabetes, diabetic kidney damage occurred more obvious. 2 patients early blood levels of IGF-1 is higher than the non-diabetic nephropathy in type 2 group and the control group. As the disease progresses, blood IGF-1 levels were also increased. Endstage diabetic nephropathy The serum IGF-1 levels highest. Description With the progression of diabetic kidney damage, blood levels of IGF-1 also will increase. 3 UAER and IGF-1, Hb A1 c, FPG, TC, TG, LDL-C, systolic blood pressure, duration was positively correlated negatively correlated with GFR, HDL-C(P<0.05). Which IGF-1 standard regression coefficient, which indicates that the blood levels of IGF-1 the greatest impact on the degree of diabetic nephropathy, further to different levels of DN conduct blood levels of IGF-1 Logistic regression analysis results indicate that higher than normal blood IGF-1value range, the occurrence of diabetes is also increasing the risk of DN. 4 by Pearson correlation analysis, serum IGF-1 and UAER, Hb A1 c, FPG, TC, TG, LDL-C, systolic blood pressure, duration was positively correlated(P<0.05), and GFR, HDL-C was negatively correlated(P<0.05).Conclusions 1 Serum IGF-1 levels increased with the severity of diabetic nephropathy positive correlation. 2 serum IGF-1 is an independent risk factor for diabetic nephropathy,it may be involved in diabetic nephropathy and development.
