| Objective:Diabetes mellitus is the psychosomatic disorder seriously harmful to human health.Anxiety and depression often comorbid with it,both of them have a vicious circle and mutually effect.Meanwhile,diabetes is a kind of chronic diseases needs to be treated,individual and diversified treatment plan almost accompanied by all diabetic patients’ life.By investigating the impacting study on anxiety and depression in type 2 diabetic sub-groups treated by three kinds of glucose-lowering scheme,in order to guide clinical operation,to provide scientific basis for improving their mental health atatus.Methods:We studied continuously 70 cases of type 2 diabetes only treated with oral hypoglycemic agents(OHA group),50 cases of type 2 diabetes only treated with insulin(INS group),and 70 cases treated with oral hypoglycemic agents combined insulin(OHA + INS group)from June 2014 to December 2014 in the Endocrinology and Metabolism in-patient department of the Second Affiliated Hospital of Nanchang University.Investigated by questionnaire on site,Self-rating Anxiety Scale(SAS) and Self-rating Depression Scale(SDS) were adopted for their emotion assessment,the general questionnaire was used to collect their information.All of these data were analyzed by SPSS 19.0.Results:1ã€The incidence of anxiety was higher in OHA+ INS group(25.71%) than INS group(18%), than OHA group(11.43%);The incidence of depression was higher in INS group(48%) than OHA+ INS group(34.29%),than OHA group(31.4%).18.42%patients had anxiety, including 14.74% with mild, and 3.68%with severe anxiety.36.84% patients had depression, including 23.68 % with mild, and 13.16 % with severe depression.Among them,the incidence of anxiety and depression comorbid condition was 15.79%,one of them was 55.26%.2ã€The scores of SAS and SDS in INS group and OHA +INS group were more significantly elevated than those in OHA group(P<0.05),but there were not differences statistically in SAS and SDS scores between INS group and OHA+ INS group(P>0.05),Compared with the national norm(SAS:29.78±10.07, SDS:33.46±8.55), the scores of SAS and SDS in each group had statistically significant.(P< 0.01).3ã€The results of logistic regression analysis indicated that the impacting factors for anxiety emotion included gender(OR:2.897,95%CI:1.320~6.361),course of diabetic disease(OR:1.761,95%CI:1.175~2.639);Both of family economical income( OR:0.374, 95%CI:0.231~0.608), course of diabetic disease( OR:1.423,95%CI:1.026~1.973)are associated impact factors for depression.Conclusion:1ã€type 2 diabetic sub-groups treated by three kinds of different glucose-lowering schemes,over half of patients existential anxiety or depression.Confirmed again the emotional disorders of anxiety and depression are common to patients with type 2diabetes;2ã€The patients with insulin treatment had a significantly higher incidence of anxiety and depression on three kinds of differernt glucose-lowering schemes,especially women,longer course of diabetic disease, lower family economical income.3 〠In clinical practice,we should pay more attention to early screening,assessment and intervention in time of diabetics on emotional disorders. |
