Objective To study the relationship and significance of mammographic Parenchymal patterns(MPP)and BI-RADS(breast imaging reporting and data system) classification for breast screening in countryside women. Methods 1. 1334 ﹙ 2619 ﹚ local countryside women underwent breast screening including mammography. The results were classified with MPP and BI-RADS classification of mammography(MAM). Breast with class 3-5 of BI-RADS or class IIIc、IVc of MPP were regarded to be in high risk.Follow up 20 months, Breast were classified to be class 1 and 2 with BI-RADS classification and regarded further to be in high risk of breast carcinoma with MMP shuld be classed with BI-RADS classification again, The relationship and followup diference of two classification methods were compared. 2. Local countryside women were screened for breast carcinoma with mammography from August 2010 to December 2010. The results were classified with BI-RADS classification. Breast were classified to be class 1and 2 according to BI-RADS were classified further with MMP. Those breast were classified to be class IIIc and IVc with MMP were regarded to be in high risk for breast cancer. They were divided into two groups and followed up for 18 months. During follow-up, women in Group Intervention received clinical intervention treatment and the others in Group non-Intervention did not. Incidence of breast carcinoma between two groups was compared after follow-up.Results 1. 2619 breasts were classed with MPP and BI-RADS.The result of each MPP class including high risk with BI-RADS class was :Ia 75/5,Ib240/3,IIa 360/6,IIb 102/1,IIIa 183/6,IIIb 156/1,IIIc 123/6,IVa531/14,IVb 750/45,IVc 99/9.The two methods had statistically significant with association analysis(P<0.05),and no statistically significant with Spearman rank correlation(P﹥0.05).207 breasts were to be class 1 and 2 with BI-RADS classification and regarded further to be in high risk of breast carcinoma with MMP,there were 6 breasts were regarded to be in high risk with with BI-RADS classification again(P<0.05);2. Among 2523 breasts which were classified to be class 1 and 2 with BI-RADS classification, 207 were found to be in high risk of breast carcinoma with MPP 139 received clinical intervention treatment and 68 breasts did not. After 18 months follow-up, none in Group Intervention and 3 women in Group non-Intervention were diagnosed breast carcinoma. The incidence of breast carcinoma between two groups were found to be different statistically(P<0.05).Conclusion The results of MPP and BI-RADS classification had association for breast screening in countryside women, but they are not much intimate. It was significance and effective of MPP combine BI-RADS be used to breast screening in countryside women.
