Objective:(1) To compare the expression of CD19+CD5+B cells and serum level of interleukin-10(IL-10) in patients with SLE, SS, RA and healthy controls, discuss the expression differences in different autoimmune disease;(2) To compare the expression of CD19+CD5+B cells and serum level of IL-10 in SLE with different disease activity, discuss the significance of the expression of CD19+CD5+B cells and serum level of IL-10 in different disease activity;(3) To compare the relation of the expression of CD19+CD5+B cells, serum level of IL-10, ESR, SLEDAI and complement, discuss the correlation between CD19+CD5+B cells and IL-10, and disease activity.Methods:Outpatient and inpatient in Second Hospital of Shanxi Medical Collage from June2013 to June 2014 were recruited, who was diagnosis with SLE. All of the patients were signed informed consent. The general condition and the clinical data of SLE patients had been collected. According to SLEDAI( systemic lupus erythematosus disease activity index) score, fifty SLE patients were divided into two groups. At the same time, the patients with SS, RA and healthy controls were picked up.First, extracting peripheral blood lymphocytes for culture in vitro, then use the FCM(Flow cytometry) detect the percentage of CD19+CD5+B cells. Analyze the variation in different autoimmune disease and SLE disease activity, and the association with ESR, SLEDAI score and complement, to study the relationship between CD19+CD5+B cells with SLE disease activity. Second, use enzyme-linked immunosorbent assay(ELISA) detect the serum level of IL-10. Analyze the variation in different autoimmune disease and SLE disease activity, and the association with ESR,SLEDAI score and complement, to study the relationship between IL-10 with SLE disease activity.All the data were analyze with SPSS 17.0.Results:(1) The percentage of CD19+CD5+B cells in patients with SLE, SS, RA and healthy controls was respectively 5.93±2.11%, 18.49±6.75%, 17.51±2.38% and 19.08±2.92%.There was statistically significance difference between the groups(P<0.05). Through pairwise comparison, the percentage of CD19+CD5+B cells in patients with SLE was prominently lower than those in the other groups(P<0.05). The serum level of IL-10 in patients with SLE, SS, RA and healthy controls was respectively 19.86±11.17 pg/ml,10.37±2.91 pg/ml, 11.76±3.29 pg/ml and 8.26±2.94 pg/ml. There was statistically significance difference between the groups(P<0.05). Through pairwise comparison, the serum level of IL-10 in patients with SLE was conspicuously higher than those in the other groups(P<0.05).(2) The percentage of CD19+CD5+B cells in low disease activity and high disease activity group was respectively 6.48±1.91% and 5.18±2.20%. The difference between the two groups had statistical significance(P=0.041). The serum level of IL-10 in low disease activity and high disease activity group was respectively 13.54±8.35 pg/ml and23.42±11.08 pg/ml. The difference between the two groups has statistical significance(P=0.002).(3) The percentage of CD19+CD5+B cells had a negative correlation with SLEDAI score(r =-0.292, P=0.049) and ESR(r=-0.090, P=0.044),but had a positive correlation with C3(r = 0.157, P=0.027). The serum level of IL-10 had a positive correlation with SLEDAI score( r = 0.322, P=0.029)and ESR(r = 0.440, P=0.002), but had a negative correlation with C3(r =-0.237, P=0.013) and C4(r =-0.324, P=0.028).Conclusions:(1) The CD19+CD5+B cells and IL-10 play an important role in the pathogenesis of SLE.(2) With the increase of disease activity, CD19+CD5+B cells in SLE patients significantly decreased, and serum IL-10 level is significantly higher.
