Objective: To explore risk factors of vertebral fragility fracture and the effect factors about different spinal fracture sites in postmenopausal women.Methods: Totally 274 asymptomatic postmenopausal women over the age of 50 were selected. Every patient underwent dual-energy X-ray absorptiometry(DXA)scans to measure the bone mineral density(BMD)of total hip,femoral neck,and lumbar spine(L1-L4).T-scores were attainted at the same time. vertebral fracture assessment(VFA) scans was done using the same DXA device to find vertebral fractures. Their general information was recorded.All patients grouped by the osteoporosis diagnostic criteria and fracture sites.Results: The VFA results showed that vertebral fragility fractures were present in 103 of 274 participants.Univariate analysis showed that age,menopause age,menopausal duration and bone mass were possibly associated with vertebral fragility fracture(P<0.05).Multivariate non-conditional logistic regression analysis indicated that menopausal duration and bone mass of total hip were risk factors of vertebral fragility fracture(P<0.05). Menopausal duration is longest,BMD of lumbar vertebrae and femoral neck are lowest in thoracolumbar vertebral fracture among three groups.Lumbar vetebrac BMD is lower in lumbar vertebrae fracture than thoractic fracture. Femoral neck BMD is lower in thoractic fracture than lumbar vertebrae fracture.BMD combined with VFA improves the diagnosis of osteoporosis increased by 7.3 percent.Conclusion: The longer of menostasis and the lower bone mass of total hip, may be important of vertebral fragility fracture. The occurrence of different parts of vertebral fracture were affected by menopausal duration,lumbar vertebrac BMD and femoral neck BMD. BMD measurement combined with VFA for postmenopausal women at same time are necessary to improve the diagnosis of vertebral fracture and osteoporosis.
