| Purposes: The research adopts the research method of evidence-based medicine to comprehensively describe domestic and foreign practice of JCI standard and research evidence, and analyzes the research evidence of influence on all aspects in medical organizations before and after the implementation of JCI to provide reference for decision-makers and policy researchers.Methods: Search 13 databases and search platforms based on network, including 5 evidence-based medicine databases, 8 comprehensive databases, and 1 official website. According to literature acceptance and removal standards, the literatures that comply with acceptance standard are selected. For acceptance literatures, assess the literature quality and extract data. The quality assessment is given scores from 2 aspects and 8 indexes. Data extraction includes basic information, applied information, and result information with 38 total projects. According to the information, International Hospital Unification Assessment and Standard is described and compared in literature feature, applied feature, and applied result. According to the information above, exact information quantification; and use multi analysis, relevant analysis, and logistic regression analysis to make comprehensive analysis.Results: ⑴The paper makes primary searches of 15,544 articles and 249 articles are included after screening. According to the quality evaluation results, that the domestic literatures are not high in quality, as for domestic literatures: the literatures of Grade A account for 0.8%(2/224), Grade B 53.1%(119/224), Grade C 46%(103/224), while as for the international literatures, the literatures of Grade A account for 8%(7/25), Grade B 92%(18/25), Grade C 0(0/25). ⑵ In terms of the literature information, as for article types in all domestic researches on basic information, experimental researches account for 33.7%, descriptive researches account for 60.6%; in terms of research regions, researches in the regions at Level 1 account for 65.1%; in the general research categories, researches on patients account for 30.9%. Within the international scope, experimental researches account for 88%, the researches from the developed countries account for 19.8% with patient researches accounting for 88% in the general categories. The domestic researches on application information conducted for management standardizing account for 58.6%, and the researches covering the broadest areas are of drug management, accounting for 14.4%; in terms of data acquisition methods, 47.6% of researches are qualitative in nature while 37% in quantitative evaluation. Among the international researches, 62% are conducted for cost effectiveness analysis, 48% of the covered areas are about the patient quality, and 96% of the data acquiring methods are of quantitative evaluation. The resulting information consists of 21 items in total, and 47.8% of the comprehensive results make it clear that the standard has a positive effect on hospital while 52.2% of the researches show that they need to be treated with caution. ⑶ The pairwise variables for comprehensive analysis of the statistical significance correlativity are year of research ~ journal publishing(in negative correlation at a coefficient of-0.1561), year of research ~ article type(in positive correlation at a coefficient of 0.2399), year of research ~ region of research(in positive correlation at a coefficient of 0.2080), year of research ~ hospital level(in negative correlation at a coefficient of-0.3739), journal publishing ~ article type(in negative correlation at a coefficient of-0.1905), nature of institution ~ region of research in positive correlation at a coefficient of 0.1624), and nature of institution ~ hospital level(in negative correlation at a coefficient of-0.2128). The final model is fitted: "Logit P=1.279a+2.735b-6.303". From the final results, there is a positive correlation between the article type and the final conclusion, but the relationship r is 0.3029 remaining between 0.3-0.5 as a low correlation.Conclusions: ⑴ There are a large number of JCI related researches at home, but they are in lower quality compared with foreign ones in methods. ⑵ The most domestic researches are conducted through observation and description and mainly come from higher income areas, suggesting that we should strengthen researches in remote areas. The domestic literature researches are still in the hospital level, based on the qualitative method. Internationally, the researches have come into society and are conducted mostly by a quantitative method, which indicates that the domestic researches need to adjust research perspectives and methods. ⑶The resulting information and the comprehensive analysis results show that the application of JCI standard does have positive effects on hospital management and patient safety, but it needs to be treated with caution. |
