The Variance Management System plays a crucial role in the management of Clinical Pathway, which involves an array of issues such as the identification, record and analysis of the variation. Compared with the well-studied and implemented clinical pathways, study on the variance management is still in its infancy. Besides, due to the absence of systematic institutions and standards for the variance management at the national level, the variance management in most domestic hospitals is merely formality, failing to play its due role in refining clinical pathways and improving medical services.Based on literary studies, this dissertation proposes and forms the concept of "VMS" through introducing the philosophy and approaches of JCI. And the fundamental elements include:identification of variation, collection or record of variation, analysis of variation, and the report and application of variation. This article also summarizes the specific practices and modes of the variance management domestically and globally, and elaborates on the mode of VMS operation, namely the circular and continuous running of PDCA.In the meantime, the author carried out in-depth interviews with managers in charge of the variance management in A hospital of Wuxi city to know the status quo of the VMS, especially the existing problems in practical operations, and put forward constructive suggestions accordingly. Such suggestions aim to facilitate domestic studies on the VMS and provide references in terms of VMS operations of CP for our counterparts and all relevant parties.
