Objective:To detect the proportion of Th17 cells and Treg cells in CD4+T cells of peripheral blood of the patients with chronic obstructive pulmonary disease(COPD) respectively and to survey the influence on them by all trans retinoic acid(ATRA),meanwhile to explore the influence and significance of the balance of Th17/Treg affactetd by ARTA in the COPD patients.Methods:To collect 23 patients with AECOPD,23 patients with COPD stable period,23 healthy smokers,23 healthy non-smokers,extracting peripheral venous blood of each object to preparae the peripheral blood mononuclear cell suspension(PBMC),and to divid them into blank group and ATRA interventing group,the flow cytometry detect the scale of Th17 cells,Treg cells in CD4+T cells of all experiment objects of each group.Results:The proportion of th17 cells in CD4+T cells in AECOPD,patients with COPD stable period,healthy smokers were higher the healthy non-smokers,particular for AECOPD,all of them were reduced by ATRA,The proportion of Treg cell in CD4+T cells in healthy smokers was higher than AECOPD,patients with COPD stable period,healthy non-smokers,all of them were heightened by ATRA.(1)the expression of Th17 cells and Treg cells in CD4+T cells of peripheral blood:the blank groups were higher than ATRA intervention groups(P < 0.05),in the blank groups,group A(6.95±2.33) was higher than group B(5.21±1.37),C(4.15±1.23),D(2.84±1.21)(P<0.05),group B was higher than group C,D(P < 0.05),group C was obvious higher than group D(P < 0.05);In ATRA intervention groups,group A(4.66±1.86) was higher than group B(2.77±1.10),C(3.10±1.28),D(1.68±0.80)(P < 0.05),between group C and group B,the result was no statistical difference(P > 0.05),and both of them were higher than group D(P < 0.05).(2)the expression of Treg cells in peripheral blood: the expression of ATRA intervention groups were more obvious than blank groups(P < 0.05), in blank groups,group C(4.64±1.36)was higher than group A(2.50±1.21,B(3.46±1.29), D(3.55±1.59)(P < 0.05),compared to group B and D,the result has no statistical significance(P > 0.05),But both of them were higher than group A(P > 0.05),in ATRA intervention groups,group C(6.28±1.44)was higher than group A(3.26±1.44),B(4.53±1.54),D(4.45±1.94)(P < 0.05),compared group B with D,the result had no statistical significance(P >0.05),and both were higher than group A(P< 0.05).Conclusions:(1)The imbalance between Treg and Th17 cells was been in peripheral blood of patients with COPD,and the extent of imbalance was connected with the severity of illness.The expression of Th17 cells was more significant in AECOPD than COPD ease period,but the change of Treg cells was contrry with Th17 cells. the expression of Th17 cells in patients with COPD was higher than smokers,non-smokers,and the expression of Treg cells was more significant in smokers than non-smokers.(2)ATRA intervention,the expression of Th17 cells of all experiment objects had lower level,while Treg cells was promoted to a certain level. So ATRA can correct the imbalance of Th17 and Treg to some extent.
