Background:Cirrhosis of the liver is a common clinical effects of whole body chronic diffuse disease, and it is the main outcome of common chronic liver disease. Hepatitis B is one of the leading cause of the. Patients with liver cirrhosis often died of the complications which conclude upper gastrointestinal bleeding, hepatic encephalopathy and liver cancer. Data indicated that compensatory cirrhosis 5-year survival rate is 84%, the decompensated phase cirrhosis 5-year survival rate is only 14%.Liver transplantation is the most effective treatment of advanced liver cirrhosis. Given the limited medical resources and expensive medical cost, internal medicine comprehensive treatment, especially to combine traditional Chinese and western medicine, playing the advantages of integral syndrome differentiation of traditional Chinese medicine, improving the patients quality of life,will be still a medical problem urgently to be solved in quite a long time. Features and advantages of cirrhosis diagnosis and treatment of Chinese medicine reflect on the individualized treatment on the basis of identifying syndrome. Clinical diagnosis and treatment of Chinese medicine often give priority to in order to experience, so identifying syndrome is still lack of unified objective standard. Metabolomics is the important component of systems biology, its real-time, dynamic and integrity can reflect the characteristics of TCM syndromes. It is a kind of effective means for exploring cirrhosis essence of syndromes and diseases in the evolution of the false or true relationship.Objective:This topic analysis the urinary metabolomics contour of hepatitis B cirrhosis liver blood stasis and the non-blood stasis based on LC-MS/MS technology. By observing the urine metabolomics characteristics of two groups, we find the potential biomarkers of Hepatitis B Cirrhosis patients with liver blood stasis. It provides ideas for the research of TCM syndrome essence.Methods:Collect 22 cases of male Hepatitis B Cirrhosis patients with liver blood stasis and 20 cases of the non-blood stasis urine specimens, pretreatment. We use LC-MS/MS technology for chromatography, mass spectrometry detection, data acquisition, and the MassLynx software to process the data, including ion pair extraction, peak alignment, peak matching, etc. We import it SIMCA-P+12.0 software for multi-dimensional statistical analysis after using 80% principle to zero. Distinguish between group differences in metabolic profile through PCA and PLS-DA methods. According to the VIP comparison between the two groups to guess the possible potential metabolites, then retrieve the biological information database. Finally, by comparing the sample two metabolic level spectrum and standard two order spectrum, ultimately determine the potential biomarkers. General information and biochemical data statistics for t test using SPSS software.Result:1、Metabolic components have obvious separation trend in Hepatitis B cirrhosis of liver blood stasis and the non-blood stasis group; Metabolites of discrete tendency also can be distinguished, group clustering very satisfied. We screen out differences metabolites, analyze qualitatively, and find out 7 potential biomarkers, such as alanine, D-glucose, glycochenodeoxycholic acid, lysophosphatidyl ethanolamine, citric acid, proline, bile acid.T test was performed and the results are statistically significant (P<0.05).2、The statistical results of general information:the serum biochemical indexes(WBC、RBC、HGB、PLT, ALT、AST、CHE、TBIL、ALB、A/G, PT, INR)differences between the two groups were not statistically different (P>0.05).Conclusion:Based on LC-MS/MS technology and multi-dimensional statistical analysis, the two groups are separated well, and we find out 7 potential biomarkers (alanine, D-glucose, glycochenodeoxycholic acid, lysophosphatidyl ethanolamine, citric acid, proline, bile acid)of Hepatitis B cirrhosis of liver blood stasis. Medical research in the future will be more rigorous, many technologies and the research conclusion of integration will be used in the diagnosis and treatment. The successful exploring of the liver blood stasis by metabolomics from the overall view, provides a new idea and method for the study of the essence of TCM syndrome.
