Objective:Recent evidence suggests that secreted frizzled-related protein (SFRP) 4 may affect β-cell function. Subjects were enrolled to investigate the relationship between serum SFRP4 levels and the first-phase of glucose-stimulated insulin secretion, glucose metabolism and inflammation.Method:In a cross-sectional clinical study,56 patients with newly diagnosed type 2 diabetes mellitus (T2DM group),52 patients with newly diagnosed impaired glucose tolerance (IGT group), and 42 normal glucose tolerant subjects (NGT group) were enrolled. Intravenous glucose tolerance test (IVGTT) were carried out and acute insulin response (AIR), the area under the curve of the first-phase (0-10min) insulin secretion (AUC) and glucose disposition index (GDI) were calculated. Fasting serum secreted frizzled-related protein 4 (SFRP4) and IL-1β were assayed by ELISA. Homeostasis model assessment for β cell function index (HOMA-β) and insulin resistance index (HOMA-IR) were calculated.Result:(1) There was a statistically significant difference of serum SFRP4 and IL-1β between the three groups and with a progressive increase from NGT to IGT groups and a highest value in the T2DM groups (P< 0.01). The AIR, AUC, GDI between the three groups with a progressive decrease from NGT to IGT groups and a lowest value in the T2DM groups (P< 0.01). (2) SFRP4 was negatively correlated with AIR, AUC, GDI, HOMA-p (P<0.01), and positively correlated with fasting plasma glucose,2h plasma glucose after glucose loading, HbAlc, IL-1β, hs-CRP (P<0.01). (3) Multiple stepwise regression analysis showed that AUC, HOMA-IR and serum IL-1β level were independently associated with SFRP4.Conclusion:The concentrations of serum SFRP4 correlated closely with the glycose metabolic disorder, the first-phase of glucose-stimulated insulin secretion and chronic low-grade inflmmation. SFRP4 may be involved in the mechanism of β cell dysfunction in type 2 diabetes mellitus.
