A Comparative Study Of TVT And TVT-O In The Treatment Of Female Stress Urinary Incontinence

Objective:To compare the curative rates, perioperative complications, operation time, hospital stays and hospitalization expenses associated with tension-free vaginal tape (TVT) procedure and Transobturator vaginal tape inside-out(TVT-O) procedure in the treatment of female stress urinary incontinence(SUI).Methods:The clinical records of 54 patients diagnosed with SUI who has undergone TVT or TVT-O form September 2009 to March 2014 in the First Affiliated Hospital of Chongqing Medical University were analyzed. TVT procedures were carried out in 18 cases and TVT-O procedures were performed in 36 cases. All procedures were operated by urologists who have at least 10 years of working experience. Mean operation time, blood loss, total hospital stays, hospitalization expenses, intraoperative and postoperative complications of 2 groups were recorded. All participants were followed for at least 6 months after surgery. The data analysis was finished by SPSS 19.0 software package. T-test was applied to the comparison of measurement data, such as the operating time, blood loss volume, the total hospital stay and hospitalization costs. The curative rate between two groups were compared by the Fisher’s exact test. P<0.05 was considered that the differences were statistically significant.Results:After comparison, there were no statistically significant differences of the patient characteristics between 2 groups(P> 0.05). The mean intraoperative blood loss of TVT group was more than of TVT-O group, and there was a statistically significant difference (P<0.05).The mean operative time and total hospital stay of TVT group was significantly longer (P<0.05).The cure rate was 88.89%(16/18) of TVT group and 91.67%(33/36) of TVT-O group and the improved rate of these two groups was 11.11%(2/18) and 8.33%(3/36), respectively. No significant differences indicated in cure rate and improved rate between two groups (P>0.05). Rates of complications had no significant difference (16.66% vs11.11%).Conclusions:TVT and TVT-O procedures were both effective strategies in the treatment of SUI. The curative efficacy between TVT and TVT-O procedures were similar. However, TVT-O had shorter operative time, less invasive, lower medical cost.
