Background:posterior lumbar interbody fusion has been considered a gold standard in the treatment of degenerative lumbar spinal instability, but the postoperative clinical problems become more and more attention.Objective:to compare the recent clinical efficacy and imageology results between Dynesys dynamic stabilization system and posterior lumbar interbody fusion (PLIF) in the treatment of patients with degenerative lumbar instability. The adjacent segment degeneration were evaluated according to the last follow-up imageology resultsMethods:A retrospective analysis on 28 cases of degenerative lumbar instability patients admitted in our department from December 2012 to May 2014 was conducted. The posterior lumbar decompression combined with vertebral pedicle dynamic stabilization system (observation group, dynesys, group A14 cases) and posterior lumbar decompression interbody fusion (control group, group B, PLIF,14 cases). Compared two group after surgery in January, march, June, December and time follow-up Visual analog pain Score (Visual Analogue Scale/Score, VAS) andOswestry disability index Score (ODI Score) and collect preoperative and postoperative lumbar anteroposterior and lateral dynamic radiographs observe the record. Evaluate adjacent segment ASD in the White judgment standard, The data were analyzed using SPSS17.0 software.Results:all patients were followed up, with an average follow-up of 18+3.4 months. Two groups of patients with postoperative 1 month,3 months,6 months, at the end of December and follow-up of VAS score, ODI score were obviously improved compared with preoperative, there were significant (P<0.05);Two groups of patients with postoperative times VAS score, ODI score comparison no statistical difference (P>0.05); observation group has ASD 1 case according to the last follow-up imageology result; Control group was noted in 4 patients, observation group has a lower incidence of ASD, statistically significant (P<0.05).Conclusion:1, The curative efficacy of Dynesys dynamic stabilization system was equivalent with that of posterior lumbar interbody fusion in the treatment of degenerative lumbar instability;Can provide lumbar spinal stability, improve the clinical symptoms;2, Dynesys dynamic stabilization system can delay the degeneration of the adjacent segments.
