The improvement of dl-3n-butyphthalide injection on cerebral blood flow incerebral infarction patients with responsible artery severe stenosis or occlusion Objective:To investigate the improvement of NBP injection on neurological function and cerebral blood flow in acute cerebral infarction patients with responsible artery severe stenosis or occlusion. Methods:40 cases of acute anterior circulation cerebral infarction who received treatment within 72 hours in Department of Neurology, the First Hospital of Jilin University during August 2014 to October 2015, were taken as test subjects. All patients were confirmed severe stenosis or occlusion in internal carotid artery or middle cerebral artery with MRA and TCD. The subjects were set into 2 groups: 25 cases as NBP treatment group and 15 cases as control group. The two groups were both treated with regular treatment(aspirin and ozagrel sodium injection), but in addition the treatment group were treated with NBP injection 25 mg, intravenously, twice daily, for 10 days. Before and after treatment, all patients underwent NIHSS 、 PWI and 3-month follow-up m RS, to observe the changes of ischemic penumbra area(IP volume=TTP delay volume﹣DWI volume) and PWI four parameters( r MTT, r TTP, r CBV, r CBF).Results:1, NIHSS and m RS : Before treatment, the score of NIHSS in two groups was with no significant difference(P>0.05); after treatment, the score of NIHSS in two groups both reduced significantly(P<0.001) and the change of NIHSS in treatment group was more obvious(P<0.01). The score of m RS in the treatment group was significantly lower than that of the control group at 3 months(P<0.05). 2, IP volume: Before treatment, the volume of IP in the two groups was with no significant difference(P> 0.05); after treatment, the volume of IP in the two groups decreased significantly(P<0.001) and the change of IP volume in treatment group was more obvious(P<0.01). 3, Cerebral blood flow parameters:(1) Before treatment, r MTT, r TTP, r CBV, r CBF in the ipsilateral side of lesion of two groups were compared with no significant difference(P> 0.05); After treatment, r MTT and r TTP in the ipsilateral side of lesion of two groups both shortened significantly, r CBV and r CBF in the ipsilateral side of lesion of two groups both increased significantly and there were respectively significant differences between two groups(P< 0.05).(2) After treatment, r MTT and r TTP in the contralateral side of lesion of two groups both shortened, r CBV and r CBF in the contralateral side of lesion of two groups both increased, but there were no significant differences between two groups(P> 0.05). Conclusion:1, NBP injection can significantly improve neurological deficit in acute cerebral infarction patients. 2, NBP injection can significantly improve cerebral blood supply in the ischemic penumbra.
