1. ObjectivesTo investigate the clinical efficacy of the management of extensive deep partial thickness burn wound(with burn size over 50% TBSA) by early dermabrasion with porcine acellular dermal matrix.2.Methods:Aretrospective study was performed to analyze the patient of extensive deep partial thickness burn between September 2009 and September 2013 in our hospital.Twenty patients underwent early dermabrasion with porcine acellular dermal matrix(group A).Twenty patients underwent early dermabrasion with nano-silver dressings(group B).Twenty patients were treated by conservative dressings(group C). Antibiotic application time, fever time, platelet declining time,morbidity and mortality,pains, and the time and quality of wound healing among the three groups were examined. The length of hospital stay were analyzed. Scar assessment was performed 3 and 12 months post-operatively with a modified Vancouver Scar Scale linked with TBSA(m VSS-TBSA).3.ResultsCompare with group B and C,the antibiotic application time,fevertime, platelet declining time were all significantlyshorter in group A(P<0.01). The rate of sepsis, MODS decreased significantly(p<0.05) and the healing time also significantly decreased in group A. The mean time in hospital of patients received early dermabrasion and porcine ADM was 28.3±7.2 days, which was significantly shorter than that of group B and C(P<0.05~0.01). The m VSS-TBSA of patients in group A was significantly lower than that of group B and C at 3 and 12 months after the injury. here was no significant difference in mortality rate between three groups(P > 0.05). The proliferation of scar was lighter ingroup A in comparison with group B and C.4.ConclusionEarly dermabrasion combined with porcine acellular dermal matrix is beneficial in the management of extensive deep second burns. It can reduce the incidence of invasive infection and complication, accelerate wound healing, decreases the length of hospital stay and improve the skin quality.
