| Objective Nanosilver dressing,porcine acellular dermal matrix dressing and nanosilver /porcine acellular dermal matrix dressing are used respectively in the wounds of deep second degree burn,comparing the differences of the wound infection,the wound healing rate,the wound healing time and the pain during dressing change,in order to explore a new type of dressing which can resist wound infection,promote wound healing and reduce pain.Method From January 2015 to January 2016,according to the inclusion and exclusion criteria,90 patients with deep second degree burn are treated consecutively,according to the order of admission,patients are devided randomly into 3 groups,each group has 30 patients.On the day of admission,take pictures and calculate the burn areas first,then take the wound secretion for bacterial culture by using the throat swab,burn wounds are treated with debridement and dermabrasion,then dressed with nanosilver dressing(group A),porcine acellular dermal matrix dressing(group B)and nanosilver /porcine acellular dermal matrix dressing(group C).On the 7th day,the 14 th and 21 st day,when replacing the dressings,take the wound secretion for bacterial culture by the throat swab again,and score the patient’s pain.On the 14 th and 21 st day,take pictures of the areas of the wounds,and calculate the wound healing rate,then record the wound healing time,any side-effects are recorded during the course of treatment.After about 6 months,patients in each group are followed up,observe the scar hyperplasia and pigmentation situation of patients after wound healing.Result 1.On the day of admission,there is no significant difference among the three groups;On the 7th day,the bacterial culture results of group C and the other two groups are significantly different,but there is no significant difference between group A and group B;on the 14 th day,there is no significant difference between the bacterial culture results of group A and group C,but there is significant difference between these two groups and group B;on the 21 st day,the wounds of group C have healed,the bacterial culture result of group A and group B are not significantly different.2.On the 14 th day,the healing rate of group C is significantly higher than group A and group B,and the healing rate of group A is minimum,there is significant difference among the three groups.;on the 21 st day,the wounds of group C have healed completely,the wound healing rate of group C is significantly higher than the other two group,and the healing rate of the group A is minimum,there is significant difference among the three groups.3.The average healing time of group A is 23.04 days,the average healing time of group B is 19.73 days,and the average healing time of group C is 16.47 days,there is significant difference among the three groups.4.On the 7th day,the pain score of group C is significantly lower than the other two groups,there is significant difference among the three groups.;on the 14 th day,the pain score of group C is the lowest,followed by group B,and the pain score of group A is the highest;on the 21 st day,the wounds of group C have healed completely,the pain score is 0 point,the pain score of the group A is significantly higher than group B,there is significant difference among the three groups.5.The assay results of the three groups are not significant abnormal,and there is no adverse reaction in the treatment.Conclusion 1.Compared with the nanosilver dressing and porcine acellular dermal matrix dressing,the anti-infective property of the nanosilver /porcine acellular dermal matrix dressing is better in the treatment of deep second degree.2.Compared with the nanosilver dressing and porcine acellular dermis matrix dressing,the wound healing rate and healing time of nanosilver/ porcine acellular dermal matrix dressing is better in the treatment of deep second degree.3.The degree of pain of the nanosilver/porcine acellular dermal matrix dressing is significantly lower than the nanosilver dressing and the porcine acellular dermal matrix dressing in the treatment of deep second degree,and the degree of pain of the nanosilver dressing is heaviest. |
