Clinical Analysis Of Relevance IPTH Level Between Left Ventricular Hypertrophy Of CAPD Patients

Objective: To analyze continuous ambulatory peritoneal dialysis patients in Intact Parathyroid hormone levels are abnormally and its influencing factors, Preliminary Study Intact Parathyroid hormone and left ventricular hypertrophy relationship. Methods:Collected from January 2014 to March 2015 in our hospital 114 patients CAPD, according to iPTH(pg/ml) levels divided into four groups: > 300 groups, 150 to 300 groups, < 150 to70 groups, < 70 group. Collect all the clinical and laboratory data; ELISA assay of plasma IL-6 and TNF-a levels. Echocardiography detected ventricular diameter indicators.Results: 1) There are significant differences in the age > 60 years and < 60 years of age and with different dialysis time LVH incidence, P < 0.05; different levels of iPTH group LVH incidence overall, the difference was significant, P < 0.05; between the two groups pairwise comparisons show, iPTH > 300 pg/ml group and iPTH 150 ~ 300 pg/ml group,the difference was statistically significant, P < 0.05. 2) Comparison of biochemical indices of each group: Ca, P have overall significant difference in 4 groups(P < 0.05); Room for further pairwise comparison results in PTH > 300 pg/ml in Ca group were lower than the other three groups; while in iPTH > 300 pg/ ml in group P were higher than the other three groups. 3) Each group IL-6, TNF-α, LVMI comparison: analysis of variance showed,iPTH> 300 pg/ml group and iPTH <70 pg/ml in group IL-6, TNF-α, LVMI(left ventricular mass index) was significantly higher iPTH 150 ~ 300 pg/ml group, the difference was statistically significant(P < 0.05); correlation analysis showed, IL-6,TNF-α, LVMI and PTH was positively correlated(P < 0.05); IL-6, TNF-α and LVMI was positively correlated(P < 0.05). 4) Multivariate logistic regression analysis: Ca, P and IL-6is CAPD patients SHPT(secondary hyperparathyroidism) independent risk factors.Conclusions: 1) CAPD patients PTH is closely related to the development of LVH; age and duration of dialysis with LVH also relevant. 2) High PTH, resulting in low iPTH areIL-6, TNF-α levels higher iPTH standards group,Its IL-6, TNF-α may be involved in the occurrence of PTH cause LVH. 3) Ca, P and IL-6 is an independent risk factor for CAPD patients with SHPT impact.
