Objective:By comparing the expression of regulatory T cell(CD4+CD25+Foxp3+ T lymphocyte or Treg cell) and T helper cell 17 (Th17 cell) in peripheral blood lymphocytes of patients before transplantation and kidney transplant recipients of 1-6 months and 6-12 months after transplantation and healthy volunteers,discuss and analysis the cellular immune state of kidney transplant recipients before and 1 year after transplantation.Methods:We collected the patients who admitted to our hospital to have kidney transplant surgery and recipients who already had kidney transplant surgery and followed up in out-patients clinic of our hospital.71 kidney transplant recipients were divided into three groups:group before kidney transplantation(group A,15 cases); group of recipients followed up for the first 1-6 months after transplantation(group B,34 cases);group of recipients followed up for the 6-12 months after transplantation(group C,22 cases);21 healthy volunteers were collected as normal group(group D). The expression percentage of CD4+CD25+Foxp3+ T cell, Th17 cell and T cell subsets in peripheral blood was determined with flow cytometry.Results:(1)The expression percentage of CD4+CD25+Foxp3+ T cells in peripheral blood lymphocyte of four groups:group A(7.55±3.77),group B(4.88±2.43),group C(5.50±1.79) and group D(7.04±1.59).The expression percentage of CD4+CD25+Foxp3+ T cell was significantly decreased in group B and group C when compared with group A and group D(P<0.01); Group B was lower when compared with group C,but the result was not statistically significant(P>0.05). (2)The expression percentage of Th17 cell in peripheral blood lymphocyte of four groups:group A(2.24±1.65),group B(1.62±1.30),group C(1.94±1.32) and group D(1.67±0.83). Compared with group A,the percentage of Th17 cells expressed in peripheral blood lymphocyte was decreased in group B and group C, the differences were not significant (P>0.05). (3)The Th17/Treg ratio of four groups:group A(0.56±0.88),group B(0.46±0.48),group C(0.37±0.30) and group D(0.25±0.15). There was no significant difference when compared(P>0.05). (4)The expression percentage of CD4+ T cell in peripheral blood lymphocyte of four groups:group A(52.65±8.69),group B(55.93±12.80),group C(57.23±14.45) and group D(54.18±11.70).When compared between them, found no significant difference(P>0.05). (5)The expression percentage of CD8+ T cell in peripheral blood lymphocyte of four groups:group A(41.69±8.26),group B(38.94±13.61),group C(34.79±10.94) and group D(37.15±10.38).There was no significant difference between them(P>0.05).(6)The CD4/CD8 ratio of four groups :group A(1.36±0.56),group B(1.67±0.80),group C(1.94±1.12) and group D(1.68±0.95). It was not significantly difference between the four groups(P>0.05). (7) In group B and C,The incidence of pneumocystis pneumonia was 7%,The onset of the average time of pneumocystis pneumonia was 96±48 days after transplantation, the maximum value and minimum value was 48 days and 160 days respectively.Conclusion:1.The immunosuppressive agents that the kidney transplant recipients administered mainly influence the expression of CD4+CD25+Foxp3+ T cells in the peripheral blood.2. The immunosuppressive agents influence the expression of Th17 cell and CD8+ T cell to a certain extent.3. The incidence of pneumocystis pneumonia in kidney transplantation recipient remained at relatively high level,most recipients were occurred in half of one year after transplantation. So we infer the decline of CD4+CD25+Foxp3+ T cell in one year after kidney transplantation may have a certain relationship with the occurrence of pneumocystis pneumonia, but need further experimental proved.
