| Premature rupture of membranes(PROM) refers to the natural rupture of membranes before labor.PROM accounts for about 10% of the pregnancy, 80%- 90%patients with PROM will be in natural labor within 24 h, 90% patients with PPROM will be labor within 7 days.Premature rupture of membranes can lead to the increase of premature birth rate, the perinatal child mortality, intrauterine infection and puerperal infection rate. PROM is one of the most common complications in the perinatal period.The most basic pathological change is inflammation.Infection and inflammation can not only weaken the fetal membrane structure, but also activate the immune system of pregnant women,and then rupture of membranes. At the same time,premature rupture of membranes can also increase the risk of placenta fetal membrane inflammation reaction.Our research group has already completed the separation between the term premature rupture of membranes and normal full-term pregnancy in patients with placenta tissue, total protein and MALDI- TOF/TOF- MS identified 16 differentially expressed proteins, including protein disulfide isomerase(protein disulfide isomerase, PDI),which is a major in mammals and yeast play roles in endoplasmic reticulum lumen mercapto- disulfide bond oxidoreductase, folding enzymes. The protein in the endoplasmic reticulum associated with protein degradation pathway, protein transportation, calcium homeostasis, virus intrusion,etc.PDIA3,also called ERp57, it is the important component of thiol protein in the family of REDOX enzyme, belongs to the endoplasmic reticulum stress signal channel protein, catalyze the formation of disulfide bond, has a molecular chaperone activity.This study detected the expression changes of PDIA3 in placenta of each patients with premature rupture of membranes, and discussed its role in the pathogenesis of PROM, It may provide a new research direction for preventing treatment of PROM.ObjectThrough studying expressions and locations of PDIA3 in the fetal membrane tissue and placenta tissue of PROM groupã€TPROM groupã€premature labour and normal late pregnancy group has differences,locate PDIA3 by immunohistochemistry in placenta and fetal membrane tissue of the four groups,this paper aims at investigating the function and relation of PDIA3 in the occurrence and process of premature rupture of membrane.Materials and methods1.MaterialsCharge between September 2013 and September 2014 to the Third Affiliated Hospital of Zhengzhou University obstetric after cesarean section delivery of 30 patients with premature rupture of membranes at term and 30 patients with premature rupture of membranes before term as experimental group. Randomly chose 30 cases of normal full-term pregnancy, 30 unplanned pregnancy in the same period admitted to hospital.2.Methods(1) Use immunohistochemical SP method to more than 120 cases of placenta and fetal membrane organization PDIA3 protein expression in positioning and half quantitative detection.(2) Use real-time fluorescent quantitative PCR(RT- PCR) method for more than120 cases of placenta and fetal membrane in the organization PDIA3 m RNA expression for testing.3.Statistics analysisStatistical analyses were performed using the SPSS version 20.0 software. All data were presented as mean ± standard deviation. Accord with normal distribution between the two way of measurement data by using T test, do not accord with normal distribution of measurement data using Mann Whitney U test. Rate express count data,comparison between the two groups by chi-square test. Significance was considered as P ﹤ 0.05.Results1. Comparison of general clinical data condition of experimental group and control groupTPROM, PPROM group, the preterm group, normal full-term pregnancy women with an average age was 27.1±3.7 years old, 27.0±4.4 years old, 26.3±2.9 years old,26.6±4.1 years old.Gestational age was 38.6±1.9 weeks, 30.2±1.3 weeks, 29.5±1.7weeks, 38.9 ±1.4 weeks.The differences between the four groups of pregnant women age had no statistical significance(P ï¹¥ 0.05);Normal full-term pregnancy group compared with TPROM gestational age group, there was no statistically significant difference(P ï¹¥ 0.05);The preterm group compared with PPROM group of gestational age, there was no statistically significant difference(P ï¹¥ 0.05).2. Expression and positioning of PDIA3 protein in placental tissue of the experimental group and control groupPDIA3 in placental tissue protein is mainly expressed in the cytoplasm, the SP dyed yellow granular, PDIA3 protein in TPROM, PPROM group, the preterm group,positive rate of normal full-term pregnancy placenta tissues were 93.33%, 90.00%,43.33%, 90.00%, TPROM group compared with normal full-term pregnancy group,PDIA3 protein expression quantity increased significantly, statistically significantdifference(P ﹤ 0.05);PPROM group compared with the preterm group, the expression of PDIA3 protein increased significantly, statistically significant difference(P ﹤ 0.05).3. Expression and positioning of PDIA3 protein in fetal membranes tissue of the experimental group and control groupIn membranes tissue, PDIA3 protein is mainly expressed in the cytoplasm, a small amount in the nucleus and the cell membrane, the SP dyed yellow granular,proteins in TPROM PDIA3, PPROM group of membranes, preterm labor, normal full-term pregnancy group in the organization positive rate were 86.67%, 83.33%,46.67%, 50.00%, TPROM group compared with normal full-term pregnancy group,PDIA3 protein expression quantity increased significantly, statistically significant difference(P ﹤ 0.05);PPROM group was compared with the preterm group, the expression of PDIA3 protein increased significantly, statistically significant difference(P ﹤ 0.05).4. Expression of PDIA3 m RNA in placental tissue of the experimental group and control groupIn the placenta tissues, PDIA3 m RNA in TPROM, PPROM group, the preterm group, relative expression in normal full-term pregnancy group are: 2.025±0.180,2.012±0.188,0.995±0.278,0.278±0.418.TPROM group was compared with normal full-term pregnancy group, PDIA3 m RNA expression quantity increased significantly,statistically significant difference(P ﹤ 0.05);PPROM group was compared with the preterm group, PDIA3 m RNA expression quantity increased significantly,statistically significant difference(P ﹤ 0.05).5. Expression of PDIA3 m RNA in fetal membranes tissue of the experimental group and control groupIn membranes tissue, PDIA3 m RNA in TPROM, PPROM group, the preterm group, relative expression in normal full-term pregnancy group are: 1.440±0.053,1.452±0.056, 1.035±0.035, 0.035±0.049.TPROM group compared with normalfull-term pregnancy group, PDIA3 m RNA expression quantity increased significantly,statistically significant difference(P ﹤ 0.05);PPROM group was compared with the preterm group, PDIA3 m RNA expression quantity increased significantly,statistically significant difference(P ﹤ 0.05).ConclusionPDIA3 protein and m RNA in patients with premature rupture of membranes in the placenta and fetal membrane expression quantity are higher in the tissue, and PDIA3 may participate a variety of ways to participate in the occurrence and development of premature rupture of fetal membrane, premature rupture of membranes may be a potential biomarker. |
