| Objective:To analyze the medical staff whose individual radiation exposure dose and occupational health examination data by research center of 125 I seeds interstitial implant treatment malignant tumor in Yunnan province. We will study health status of the medical staff that was exposured under the radiation dose and provide the basis for improving medical radiation protection measures. Eventually, ensure occupational health and safety of medical staff after 125I seeds interstitial implantation.Methods:This is a cross-sectional study.36 medical staff who have close contact with the patient for 125I seeds interstitial implantation were involved in the 125I seeds group.36 medical staff who have exposed to radiation were involved in the radiation group. The control group included 36 medical staff who have never exposed to radiation. The occupational health examination results from the three groups were analyzed statistically, respectively. In addition, individual effective dose from the 125I seeds group and radiation group was comparative analyzed in 2014.Using Excel and SPSS17.0 to record and analyze the data. It also statically describes the quantitative data through the mean and median. Qualitative data are described by the frequency. The annual radiation dose comparison among different group, sex, occupational exposure time and careers is using t test and nonparametric test. We are using the variance and nonparametric tests to comparative analysis WBC count, hemoglobin content, platelet count, alanine aminotransferase, aspartate transaminase, creatinine and blood urea nitrogen. Comparison of the lens opacity and electrocardiogram should use chi square test.Results:1. Individual dose exposure conditionThe average annual dosage of 36 medical staff in 125I seeds group were (0.48±0.39) mSv/a, annual collective effective dose for 17.19 mSv, accounting for 94.44 percent of medical staff whose total dosage was less than 1mSv. The average annual dosage of 36 medical staff in radiation group were (0.61±0.46) mSv, annual collective effective dose for 21.96 mSv, accounting for 72.22 percent of medical staff whose total dosage was less than 1mSv. Two groups of effective dose were far lower than 20 mSv/a of the radiation workers annual effective dose limit prescribed by the state. They were belong to low dose exposure. No significant statistical difference was found the average annual dosage of the different gender, different dose exposure time and position of different.2. The impact on health by individual exposure dosagePeripheral blood picture results were found that the average of leucocyte among 125I seeds group, radiation group and control group were separately (5.76±1.38)x 109/L, (5.58±1.73)×109/L and (6.71±1.46)×109/L. The incidences of abnormity were separately 16.67%,16.67% and 2.78%. The value of leucocyte of the 125I seeds group and radiation group were lower than the control group (P< 0.05). Abnormal detection rate is significantly higher than the control group (P<0.05). The hemoglobin values of the 125I seeds group and radiation group are lower than the control group (P<0.05).Results of liver function test shows that alanine aminotransferase and aspartate transaminase mean value were within the normal range in the 125I seeds group, radiation group and control group. The average aspartate transaminase of the three groups was separately (24.64±7.63)u/L, (30.33±18.12)u/L and (22.81±4.90)u/L. The value of aspartate transaminase among the different groups,40 years old group and dose exposure time more than 15 years group have statistical significances. It’s statistically significant among the average of alanine aminotransferase in the dose exposure time less than 5 years group.Results of renal function test shows that Cr and BUN mean value were within the normal range in the 125I seeds group, radiation group and control group. The Cr average of three groups was separately (88.53±8.88)umol/L, (87.39±9.23) umol/L and (69.78±14.19)umol/L. It’s statistically significant the average of Cr among in the different groups, genders, age, exposure time between 5 and 15 years and more than 15 years group (P<0.05).Eye exam results shows that 4 medical staff (11.11%) have been found lens opacity in the 125I seeds group. Radiation group has 2 medical staff (5.56%) be checked out lens opacity. The control group has 1 medical staff (2.78%) be checked out lens opacity. The lens opacity rate in the internal 40 years old is higher than others. The lens opacity rate in the exposure time more than 15 year old group is higher than others.ECG results shows that 10 medical staff had abnormity ECG in the 125I seeds group. Mainly for the abnormal electrocardiogram variety was sinus bradycardia that accounting for 20 percent of the abnormal electrocardiogram.Conclusions:1. The medical staff after 125I seeds implantation not subjected to severe radiation exposure that individual annual effective dose significantly lower than 20 mSv/a that the annual effective dose be limited by the state.2. The radiation working environment is safe of the research center that 125I seeds interstitial implant treatment of malignant tumor in Yunnan province.3. Radiation exposure after 125I seeds implantation to medical personnel influence on peripheral blood with no significant difference between conventional X-ray radiation exposure, may be associated with the small sample size.4. The radiation exposure dose after 125I seeds implantation to medical personnel liver and kidney function and intraocular lens no obvious damage, abnormal ECG results with ionizing radiation is not relevant. |
