Objective to observe the expression of Th17 cells, IL-17 in peripheral blood and bronchoalveolar lavage fluid from patients with chronic obstructive pulmonary disease, and analysis of correlation between the level of Th17 cells and lung function, the level of Th17 cells and a 6-minute walk test (6minute walktest,6MWT), to explore the pathogenesis of chronic obstructive pulmonary disease.Method 38 patients with acute exacerbations of chronic obstructive pulmonary disease were as the group A, There are 12 patients from the group A application for alveolar lavage Bronchoscopy examination.12 patients with chronic cough clinic, lung Ventilation function normal,Bronchial dilation test-negative, and chest radiography normal as group B.The proportion of TH17 cells in peripheral blood and bronchoalveolar lavage fluid was determined by flow cytometry. The level of IL-17 was determined by ELISA (enzyme-linked immunosorbent assay, ELISA). All People were Finished pulmonary function tests and the 6MWT.Results Comparison between two groups:12 pations who application Bronchoscopy from the group A Comparison with those in group B for Each standard.Percentages of Th17 cells in peripheral blood and bronchoalveolar lavage fluid from group A were significantly higher than those in group B (blood in Group A:5.67±2.54%, Group B:1.03±0.31%, bronchoalveolar lavage fluid from group A:5.92±1.73%, bronchoalveolar lavage fluid in group B:1.28 ±0.33%, p<0.05), the difference was statistically significant.the level of IL-17 from group A were significantly higher than those in group B (blood in group A: 3.738±2.437pg/ml, Group B:0.757±0.551 pg/ml, bronchoalveolar lavage fluid in group A:2.775±2.087 pg/mL,Group B:0.547±0.469pg/mL,p<0.05)the difference was statistically significant. The FEV1 percentage of predicted value in group A were significantly lower than group B (Group A:49.5±10.3%, Group B:83.7±3.7%,p<0.05), the difference was statistically significant.the Results of 6MWT of group A were significantly lower than group B (Group A: 237±49m, group B:511±15m p<0.05), the difference was statistically significant.Correlation analysis:Percentages of peripheral Thl7 cells in peripheral blood of 38 patients with COPD from group A was reversely correlated with The FEV1 percentage of predicted value (r=-0.475,P<0.05), and was reversely correlated with the 6MWT (r=-0.395,P<0.05). Percentages of peripheral Th17 cells in bronchoalveolar lavage fluid of 12 patients who application Bronchoscopy from the group A was reversely correlated with The FEV1 percentage of predicted value (r=-0.641,P<0.05), and was reversely correlated with the 6MWT:(r=-0.609,P<0.05).Conclusion The expression of Th17 cells and IL-17 in peripheral blood and bronchoalveolar lavage fluid from patients with chronic obstructive pulmonary disease increased. Percentages of peripheral Th17 cells was reversely correlated with the FEV1 percentage of predicted value, and was reversely correlated with the 6MWT. Experimental results tip there are close Relationship between Th17 cell and COPD disease, they play important of regulation role in the inflammation reaction of COPD.
