| Objective : To observe the clinical data of 1055 patients with totally Implantable venous-access ports(TIVAPs), compare of the clinical using effect of TIVAPs among different methods and analyzes the reasons of the postoperative complication and study the feasibility of TIVAPs among different methods.Methods : From Mar. 2011 to Jun. 2015,a total of 1055 patients with TIVAPs were included in this study. all patients divided into three groups according to the surgery approaches methods, 163 cases were implanted through internal jugular vein, 576 cases were implanted through subclavian vein,and other 316 cases were implanted through right brachiocephalic vein. compare the clinical using effect of TIVAPs among different methods and analyzes the reasons of the postoperative complication between three groups.Results: In this study, all patients surgical success rate is 100%, the median follow-up time was 18 months(4-38 months), the puncture success rate of right brachiocephalic vein implanted group is 97.15%(307/316),higher than 89.57%(146/163)of internal jugular vein implanted group and 93.23%(537/576)of subclavian vein implanted group,the difference was statistically significant(P < 0.05);By the end of the study,the indwelling time of internal jugular vein implanted group is 7.7(9.20 ± 6.39) months,the subclavian vein implanted group is 8.1(10.73 ±7. 32) months,the right brachiocephalic vein implanted group is 8.3(10.21±5.33)months,respectively,there is no significant statistical difference(P>0.05)。Intraoperative complications is included:Pneumothorax 4 cases(0.38%), arteriopuncture 19 cases(1.80%), intraoperative complications difference between three groups have no statistical difference(P >0.05).The postoperative complication rate is 5.59%(59/1055 例),including hematoma 2 cases〠incision infection 7 casesã€Other causes of poor infusion 50 cases,the postoperative complication rate of internal jugular vein implanted group was 3.68%( 6/163), subclavian vein implanted group is 7.28%( 41 /576), the right brachiocephalic vein implanted group is 3.79%(12/316), There was difference in rate of subclavian vein implanted group between the right brachiocephalic vein group(P<0.05)。Conclusion: The study suggests that the both way of different operation methods of TIVAP are safe and reliable for long term intermittent venous a ccess. The study suggested the right brachiocephalic vein implanted group has a slight advantage over the group of subclavian vein implanted and internal jugular vein implanted, when it comes to develop the patients’ living quality. |
