Objective:To observe the clinical effects on Osteoking with extracorporeal shock wave for the treatment of early femoral head necrosis.Methods:During November 2014 to December 2015, there were 45 cases(59 hips) have be selected in the research in 175 Hospital of People’s Liberation Army, who was diagnosised as femoral head necrosis. All of patients were randomly divided into three groups, a group of Osteoking, a group of Shock Wave, and a union group. The stage of ARCO of the three groups were:the group of Osteoking has 7 cases of stage Ⅰ and 9 cases of stage Ⅱ, the union group has 7 cases of stage Ⅰ and 7 cases of stage Ⅱ, the group of Shock Wave has 7 cases of stage Ⅰ and 8 cases of stage Ⅱ.The therapy method of Extracorporeal Shock Wave was:the operation should be in the center of region of femoral head necrosis, which aim to avoid femoral artery and vein, and the pressure is set to 3 bar, the impact frequency is 100 times/min, the impact number were 2000 times. The patient who accepted the treatment of extracorporeal shock wave will have one time two days, five times as a course. Everyone of the patients should accept two courses which has one month between them. Then the patients will be asked to have a rest in bed through two weeks, after that walk with crutches four weeks. The taking way of Osteoking is to have 25ml after dinner every two days. Everyone of the taking patients should accept six courses which one of the six courses through twelve days. The patients of the group of Osteoking will take Osteoking as soon as they enrolled, every patients of the union group will take Osteoking after accepting shock wave treatment, the taking way of Osteoking will as usual as above when the course of shock wave treatment was ended. Three groups will accept X-ray and MRI on hips both before and after treatment, we will assess the Harris score of hip, the ratio of the area of femoral head necrosis and the level of hip when on the time of nine months after treatment. The three groups’ data was analysised by SPSS 18.0.Results:Before the treatment, there was no significant difference between the three groups of patients in gender, age, etiology, Harris score, ARCO staging, the rate of the area of femoral head necrosis, etc. After nine months of treatment, there were significant differences between the three groups in the Harris score and the level of hip, the union group was both better than the other two groups. After nine months of treatment, the ratio of the area of femoral head necrosis of union group was reduced to average 15.14% from preoperative average 34.07%. the ratio of the area of femoral head necrosis of the group of Osteoking was reduced to average 21.06% from preoperative average 35.13%, the ratio of the area of femoral head necrosis of the group of Shock Wave was reduced to average 21.93% from preoperative average 34.47%. After nine months of treatment, there were significant differences between the three groups in the ratio of the area of femoral head necrosis.Conclusion:The short-term effect on Osteoking with extracorporeal shock wave for the treatment of early femoral head necrosis is significant, effective to relieve the pain of hip and improve the function of hip, also helpful to decrease the area of femoral head necrosis.
