| Objective: to observe the clinical effect of tiaoshentongbi decoction combined with extracorporeal shock wave on patients with early-and medium-term alcoholic femoral head necrosis(AIONFH)and analyze its possible mechanism.Methods: thirty-seven patients with alcoholic femoral head necrosis from the outpatient department of guangxi bone injury hospital were selected.Patients meeting the inclusion criteria were divided into the experimental group(n = 19)and the control group(n = 18)according to the random number table method.The experimental group was given tiaoshentongbi decoction combined with extracorporeal shock wave,and tiaoshentongbi decoction was given once a day.The extracorporeal shock wave was given once a week for 4 weeks according to the treatment plan.The control group was given atorvastatin orally 10mg/ day and alendronate once a week for 4weeks.Three courses of treatment were given according to the design.The clinical efficacy of the treatment regimen for alcoholic femoral head necrosis was analyzed by comparing the Harris scores and imaging evaluation before and after treatment at 3,6 and the last follow-up.Results: in this study,a total of 37 patients met the criteria,and 9 patients received total hip arthroplasty due to femoral head collapse,with a head retention rate of 77.5%.At the last follow-up,Harris score was 89.74 2.21,higher than 71.97 2.91 before treatment,and the difference was statistically significant(P<0.05).The experimental group was significantly lower than the control group(P<0.05),and the difference was statistically significant.Imaging stability rate: 89.74%,77.76%,the experimental group is better than the control group.Conclusion: tiaoshentongbi decoction combined with extracorporeal shockwave therapy for alcoholic early-middle stage femoral head necrosis is effective in improving hip function score and delaying femoral head collapse.It is suitable for young patients with ARCO stage Ⅰ and stage Ⅱ. |
