Posterior circulation ischemic vertigo has become a major common disease, seriously affecting the people’s quality of life. Many patients with dizziness as the main symptom are often accompanied with nausea, headache, chest tightness and other symptoms. In the field of Western medicine, the symptoms are generally considered to result from vertebral basilar artery insufficiency. Western medicine treatment options are mostly anti-platelet aggregation,anticoagulation,thrombolysis, intervention, nerve nutrition, mainly improve posterior circulation perfusion, promote brain nerves recovery. Chinese medicine acupuncture treatment of vertigo has currently a lot of clinical researches. My tutor, the director Zhongchao Wu on the basis of many years clinical experience, mostly considers the pathogenesis of vertigo to be liver Yang exuberance, wind evil, meridian obstruction. The principle of treatment focuses on stabilizing liver Yang, expelling wind, and taking into account the local acupoints.Selecting Yifeng and Fengchi, Fengfu as the neck "three Feng points"to treat vertigo was better. This clinical study is to observe acupuncture clinical efficacy of post circulation ischemic vertigo in contrast to Western medicine treatment options,through selecting the neck "Three Feng Points" and other auxiliary point.Research objectiveThis project aims to investigate the clinical efficacy and safety of acupuncturing the neck "three Feng points"in the treatment of posterior circulation ischemic vertigo. Looking for safer,more stable and more effective Chinese medicine treatment method than western medicine.Research methodAccording to the diagnostic criteria of traditional Chinese medicine and Western medicine diagnostic criteria for posterior circulation ischemic vertigo, adopting randomized(Random number table method)controlled design. We divided the patients into the treatment group and the control group. The treatment group consisted of 46 cases, and 45 cases in the control group. The treatment group received the treatment of acupuncturing Yifeng, Fengchi, Fengfu and other acupoints based on syndrome differentiation, and are given oral aspirin 100mg once daily. The control group were treated with flunarizine 5mg once every night, aspirin 100mg once daily.The period of a course was 14 days, and the follow-up happened in 2 weeks after treatment.We observed the changes of TCM syndrome,TCD vertebral-basilar artery blood flow velocity and pulsatility index and hematocrit before and after the treatment. The objective is to compare and evaluate the clinical efficacy and recurrence after discontinuation of treatment.The results of the study1 Comparison with the total effects of the two groups, the total effective rate of the treatment group was 93.5%. The total efficiency of the control group was 86.7%. The treatment group was superior to the control group, two groups have statistically significant difference (P=0.000, P<0.05).2 The TCM symptom score of the two groups after treatment decreased significantly. The TCM symptoms total score and different symptom score comparison showed statistically significant difference (P<0.01), indicating that the two groups have all good effect.The TCM symptom total score of the two groups after the treatment all decreased.Besides,the TCM symptoms total score difference has statistically significant difference (P<0.01). The symptom total score of the treatment group decreased more than that of control group, the effect of the treatment group was significantly better than the control group. After treatment, the traditional Chinese medicine symptom score of the treatment group decreased obviously more than that of the control group.In the degree of dizziness, vertigo frequency and duration, headache, nausea, insomnia, palpitation, weak waist, fatigue and other symptoms,the two groups has highly statistically significant difference (P<0.01). The effect of the treatment group was superior to that of the control group in the regard of tinnitus, two groups have statistically significant difference (P<0.05). The two group in the treatment of vomiting, chest tightness have a positive effect, but have not any statistically significant difference (P>0.05).3 After the treatment,the TCD results of the two groups were improved.The comparison of the TCD results of the two groups before and after treatment have all highly statistically significant difference (P<0.01), indicating that the treatment group and the control group were significantly improved in the vertebro basilar artery insufficiency, increasing the flow of blood in the brain. After treatment, the TCD results are better than the control group.The comparison of TCD results difference of the two groups before and after treatment has highly statistically significant difference (P<0.01). The hematocrit results after treatment were improved.The comparison of the hematocrit results of two groups before and after treatment have highly statistically significant difference (P<0.01). Before and after treatment,the comparison of hematocrit results difference of the two groups have highly statistically significant difference (P<0.01), shows that the treatment group and the control group can significantly lower the hematocrit, reduce blood viscosity, increase cerebral blood flow.At the same time,the effect of the treatment group is better than that of the control group.4 The two groups were followed up for two weeks after discontinuation of treatment. In the treatment group, the recurrence rate was 13.04%. In the control group, the recurrence rate was 40.00%, the difference between the two groups has statistically significance (P= 0.026, P<0.05), indicating that the recurrence rate of the treatment group was significantly lower than that of the control group.Research conclusion1 Acupuncturing the neck "three Feng points "in the treatment of Posterior circulation ischemic vertigo is superior to western medicine flunarizine,and is more effective,more stability,has good clinical efficacy for the improvement of vertigo and systemic symptoms.2 Acupuncturing the neck "three Feng points "significantly improved vertebral basilar artery insufficiency, lowered the hematocrit, reduced blood viscosity, increased the flow of blood in the brain, and is superior to western medicine flunarizine.3 The recurrence rate of acupuncturing the neck "three Feng points"in the treatment of posterior circulation ischemic vertigo was significantly lower than that of Western medicine flunarizine.4 Acupuncturing the neck "three Feng points " has better clinical efficacy. Syncope, broken needle, hematoma, infection and other adverse reactions did not appear during acupuncture treatment.
