Summarization Of Prof. Yao Zupei’s Experience On Clinical Treatment Of Palpitation Based On Syndrome Differentiation

Objective:By collecting and sorting out prescriptions and medications based on syndrome differentiation from cases of palpitation treated by Prof. Yao Zupei and then to conduct analysis and induction, for summarizing his experience on knowledge of etiology and pathogenesis, syndrome differentiation and clinical prescriptions and medications of palpitation.Methods:A total of 60 outpatients and inpatients with palpitation of Nantong TCM hospital that met inclusion criteria were chose as research object. Detailed diagnosis processes of Prof. Yao were persistent recorded for 12 weeks. Curative effect of palpitation symptoms and TCM syndrome in patients before and after treatment was assessed by TCM syndrome scores table. To do statistical measures of clinical effective rates of clinical manifestations and electrocardiogram (ECG) changes in patients with palpitation before and after the treatment. In addition, clinical types of syndrome differentiation of collected cases were classified. Classification and frequency of herbs in the first prescription made by Prof. Yao were also being statistically analyzed.Results:Scores of palpitation symptoms and TCM syndrome table in 60 cases after treatment were both obviously reduced than before the treatment with a significant difference (both P<0.01). Total effective rate of clinical treatment of palpitation patients was 91.67%. The total effective rate of ECG after comparatively analyzing after treatment was 85.00%. There were four types of syndrome differentiation in 60 patients:stagnation of liver qi and inquietude of heart spirit (n=38,63.33%), asthenia of heart qi and malnutrition of the heart (n=11,18.33%), deficiency of qi and yin and disturbance of the heart (n=6,10.00%), stagnation of phlegm and disharmony of the heart (n=5,8.33%). Statistical data of classification and frequency of herbs in the first prescription from 60 cases showed that the total number of herbs was 135, among which herbs accounts for more than half was followed by cortex albiziae, raw fossil fragments, raw concha ostreae, radix curcumae, radix bupleuri, raw radix paeoniae alba, salvia miltiorrhiza, fried fructus aurantii and rhizoma cyperi that mainly belonging to herbs with function of soothing the liver and relieving depression, tranquilize mind and promoting blood circulation to remove blood stasis.Conclusions1. The curative effect of treating palpitation based on syndrome differentiation by Prof. Yao Zupei was extraordinary, at the same time improving main symptoms and accompanied symptoms of palpitation obviously.2. Through comprehensively analyzing information of four diagnostic methods, focusing on the relationship between the liver and heart, Prof. Yao Zupei put forward that the key pathogenesis of palpitation was stagnation of liver qi and disharmony of the qi and blood, and it was important to emphasize on soothing the liver and relieving depression, regulating qi and blood and tranquilizing mind and relieving fright. Therefore, herbs with function of soothing the liver and relieving depression, tranquilize mind and promoting blood circulation to remove blood stasis were always chose in clinic, and making addition and subtraction with symptoms.
