Celecoxib And Thunder Fire Moxibustion Treatment Of Knee Osteoarthritis Efficacy Difference Evaluation (Cross-over Design)

Purpose:This study is working on improving the efficacy of celecoxib and thunder fire moxibustion for treatment of knee osteoarthritis, and increasing side effects of medicine and inconvenience of thunder fire moxibustion, and improving patient compliance. It can provide a treatment basis for clinicians to select drugs or thunder fire moxibustion treatment to knee osteoarthritis as well.Methods:In January-August 2015, Jiangsu Province Hospital of Acupuncture rehabilitation clinic and Orthopaedics and Traumatology 33 cases of knee osteoarthritis patients for the study, is divided into A group and B group. Each patient has washout period about a week before treatment, that means no treatment a week. Then patients of A group is treated by celecoxib for the first 2 weeks, namely celecoxib 0.2g orally, once daily for 2 weeks; next the group has washout period for a week, any treatment related to KOA will not be; then the thunder fire moxibustion treatment is used for the next 2 weeks,4 times a week,30 minutes each time. And the B group is treated by thunder fire moxibustion for the first 2 weeks,4 times a week,30 minutes each time; it still has washout period for a week; then the B group get celecoxib treatment, namely celecoxib 0.2g orally, once daily for 2 weeks. After the end of all treatments, two groups have follow-up time at 1 week,2 week and 4 weeks. Before and after treatment, the condition changes of the two groups and the difference of curative effect in two groups were compared, by VAS score and WOMAC score as the main observation indexes. The safety of two treatments were compared by blood routine, liver function (AST, ALT) and renal function (Scr) for the safety evaluation indexes.Results:1. After the end of the first stage of treatment, VAS scores of the two groups were lower than before treatment (P<0.05); and the WOMAC scores of A group oral celecoxib were higher than B group of thunder fire moxibustion (P<0.05); WOMAC scores of pain, the degree of difficulty of life and the overall scores of group A were lower than before treatment (P<0.05), although the stiffness scores was lower than before treatment, the difference was not statistically significant (P>0.05); the WOMAC scores of group B scores were lower than before treatment (P<0.05); compared with the two groups, the B group was lower than the A group (P<0.05).2. After 1 week of washout period, VAS scores and WOMAC scores of A group were lower than before treatment (P<0.05), but it didn’t have significant differences (P>0.05); the VAS scores and the WOMAC scores of B group were lower than before treatment (P<0.05); comparing the two groups, scores of B groups were lower than the scores of A group obviously(P<0.05).3. After the end of the second stage of treatment, the two groups of VAS scores and WOMAC scores were lower than before treatment obviously (P<0.05), but the both scores were no statistically significant difference (P>0.05). At the follow-up of 1 week,2 weeks,4 weeks, the scores of group A and group B were lower than those before treatment (P<0.05), but the both scores were still no statistically significant difference (P>0.05).4. Before and after the examination, the blood routine, liver function (AST, ALT) and renal function (Scr) of two subjects groups were not significantly abnormal or changes, moreover adverse reactions did not occur in gastrointestinal tract.Conclusion:Using thunder fire moxibustion can effectively improve the pain, stiff joints and difficult life which the KOA casue firstly, and the curative effect can be maintained for a period of time. In the short term, curative effect about alternate use of thunder fire moxibustion and NSAIDs to treat KOA can maintain to ensure effectiveness, and it’s has high security.
