Objective In order to find a new immune therapeutic targets for carotid atherosclerosis,we investigated the relationship between the degree of carotid atherosclerosis, the stability of carotid atherosclerotic plaque and dendritic cell.In this way,we can find a new immune therapeutic targets for carotid atherosclerosis.Method In the Department of Neurology of Tianjin Huanhu Hospital,we continuously elected 200 patients who were suffering from ischemic cerebrovascular diseases or not from November, 2013 to March 2015.All of them underwent CVUS、TCD or CTA to reveal the presence of carotid artery sclerosis of companion or not.They were divided into four groups: early carotid sclerosis group,mild-to-moderate carotid atherosclerosis, severe carotid atherosclerosis, stroke associated with atherosclerosis, 50 cases in each group.And then we select 50 controls whose baseline data that matches the 200 elected patients.We used Flow cytometric method to test the proportion of m DCs and p DCs in peripheral blood.We used ELISA method to detect IL- 12, IL- 18,IFNαof peripheral blood plasma.Then we evaluated the differences between groups, the difference between m DCs,p DCs,IL- 12, IL- 18, the peripheral blood and peripheral blood plasma IL- 12, IL- 18,IFNαof peripheral blood plasma.According to results of the CVUS、TCD or CTA, patients blonged to mild-to-moderate carotid artery atherosclerosis and severe carotid atherosclerosis groups were divided into stable plaque group and unstable plaque group.Then we analysis difference of proportion of m DCs between the two groups.On the other hand,we choose 43 CEA patients.According to the result of pathology files, 43 CEA patients were divided into stable plaque group and unstable plaque group.At the same time,we also test m DCs,p DCs of peripheral venous blood before CEA surgeries and carotid blood during the sugeries.Then we evaluate the migration of dendritic cells and the relationship between carotid atherosclerotic plaque stability.Results Among the carotid atherosclerosis patients,ratio of m DCs, p DCs in peripheral blood was decreased, more obviously loss in m DCs.As the degree of carotid artery atherosclerosis is aggravating, m DCs ratio drops.Its change was statistically significant(P < 0.01).However,changes of p DCs ratio were no statistical significance between groups. When acute cerebral infarction occured in carotid atherosclerosis patients, ratio of m DCs declined further. On this basis, further study of m DCs, p DCs function proceeded.The plasma levels of IL- 12, IL- 18 were no significant difference(P<0.05).However,the plasma levels of IFNαbetween the groups was significant difference,as the degree of atherosclerosis is aggravating.Among unstable plaque group patients,m DC were significantly reduced in the stable plaque group(P <0.05). IHC results showed postoperative stripped plaques HLA- DR+ cells, CD11c+cells, and CD123 positive cells in unstable plaques significantly more stable plaque group(the average density of 22.4±2.04 / HPFvs19.5±1.74 / HPF,P < 0.05;6.5±1.98/ HPFvs4.6±1.05 / HPF, P < 0.05;5.2±2.13 / HPFvs3.2±1.27 / HPF,P < 0.05).Conclusion Dendritic cell distribution proportion and functional changes associated with the degree of atherosclerosis. And the migration of dendritic cells led to the development of atherosclerosis, and migration levels associated with plaque instability.The proportion of peripheral blood dendritic cells can be used as biological markers for early atherosclerosis of carotid artery atherosclerosis provides the basis for new immunotherapy targets.
