Objective: To provide some guidance for further and better treatment in unilateral cryptorchidism through understand the changes between testis and contralateral testicular.Methods: To make futher review from the tissue cytological changes of bilateral testicular through the following aspects. The testicular damage for the contralateral testicular compensatory hypertrophy; the changes of endocrine hormone level in the body; the investigation of the adult fertility and the testicular cancer risk assessment at later period by consulting a large number of domestic and foreign about the document and review of cryptorchidism.Results: As organ in pairs, one side of testicular impaired, and the other side of testicular will change tissue structure through the regulation of neural. And humoral system occur compensatory sex change, especially the participation of the hypothalamus- pituitary gonadal axis. Impaired cryptorchidism makes decrease the steroid hormones from the testis in total, so the needs can’t be met of children growth and development. Meanwhile the positive feedback effect secretion of follicle-stimulating hormone(FSH) and luteinizing hormone(LH) from anterior pituitary. FSH can work on the health side testicle,which makes sertoli cells and leydig cells mature and proliferate, consequently, making increase volume and weight of the contralateral testicular. LH and FSH give play to similar functions together.The process of primitive germ cells transform mature sperm cells barriered or delay can also occur similar phenomenon within the contralateral testes. Resulting in abnormal sperm count, shape and vitality, increase the risk of infertility at the same time, the risk of canceration also can be increased. A mass of studies have confirmed that the age work surgical operation of cryptorchidism, before 2-years-old children, could reduce the risk of canceration. And there is no evident difference with semen quality and normal male fertility after grow up.Conclusion: Bilateral testes of unilateral cryptorchidism interact with each other, and through the interactions of the neural and humoral regulation to maintain a stable internal environment, in favour of the normal growth and development of children. In cryptorchidism screening, early detection, early treatment, and 2 years old before surgery can achieve good prognosis.
