Objective: Through undergoing dynamic contrast-enhanced MRI(dynamic contrast-enhanced MRI, DCE-MRI) before operation to provide quantitative basis for the differential diagnosis of high-grade gliomas and metastasisMaterial and Methods: Twenty –seven cases of High Grade gliomas and forty-six cases of metastests were enrolled. The pharmacokinetic model of was used to calculate the datas, the tissue enhancement-time curve and pharmacokinetic parameters(volume transfer constant, Ktrans, extracellular extravascular volume fraction, Ve, and blood plasma fraction, Vp) of the lesion, solid components were obtained. Ktrans value, Ve value and Vp value were compared to judge whether there were significant differences between the two kinds of tumors.Results: The values of Ktrans、Ve and Vp in the parenchyma of HGG and in the metastases were not statistically significant(P >0.05).There was a significant difference between the Ktrans values、Ve values in the peritumoral edema of HGG and cerebral metastases(P<0.05),The Vp values in the peritumoral edema of HGG and metastases were not statistically significant(P >0.05).Conclusion: Ktrans values、Ve values in the peritumoral edema may be useful in the differentiating cerebral metastases between HGG,DCE-MRI can effectively distinguish high grade glioma from metastases through the quantitative analysis.
