Atypical antipsychotics have replaced typical antipsychotic drugs as first-line drugs for psychotic disorders, and won the approval of the The Food and Drug Administration for the treatment of acute manic, and gradually applied to the treatment of difficult-to-treat depression and anxiety disorders [1]. In consideration of the growing application of atypical antipsychotics in pregnancy[2],how to ensure the medication safety of atypical antipsychotics in pregnancy has become an increasingly urgent problem needs to be resolved in clinical practice.The Food and Drug Administration recently updated the regulation of antipsychotics in late trimester pregnancy, which claims enough information towards the potential risk of fetal toxicity and physical withdrawal symptoms to ensure drug safety of antipsychotic drugs in pregnancy. The Food and Drug Administration has made a classification of drugs during pregnancy.Products are classified under one of five letter categories--A, B,C, D, and X.A:Adequate,well-controlled studies in pregnant women have not shown an increased risk of fetal abnormalities.B:Animal studies have revealed no evidence of harm to the fetus, however, there are no adequate and well-controlled studies in pregnant women,or animal studies have shown an adverse effect, but adequate and well-controlled studies inpregnant women have failed to demonstrate a risk to the fetus.C:Animal studies have shown an adverse effect and there are no adequate and well-controlled studies in pregnant women.or no animal studies have been conducted and there are no adequate and well-controlled studies in pregnant women.D:Studies, adequate well-controlled or observational, in pregnant women have demonstrated a risk to the fetus.However,the benefits of therapy may outweigh the potential risk.X:Studies, adequate well-controlled or observational, in animals or pregnant women have demonstrated positive evidence of fetal abnormalities. The use of the product is contraindicated in women who are or may become pregnant. The truth is,Atypical antipsychotics are all C level except for clozapine(B level)used in pregnancy.There are few studies on rational use of atypical antipsychotics in clinical practise, and the results are different [3]. Therefore,it is necessarysy to deliver system analysises and comprehensive evaluations for the safe use of atypical antipsychotics during pregnancy.This article will review around teratogenicity, pregnancy complications, postpartum diseases and adverse reactions,growth development to illustrate the safe use of atypical antipsychotics during pregnancy, and provide relatively reliable reference for clinical practice.
