Objective: The distal radial fracture is one of the most frequent fracture in the orthopedic clinic. The middle-aged and old-aged people are susceptible to it. With reform and openness and development of real our country society, the crowd. An investigation was conducted in order to study and compare the clinical effect of Splint immobility and External fixator on the patients with instable distal radial fracture.Methods: This article reviewed the distal radial fracture in both ancient literature and modern medical research systematically. Summarized the etiology, pathology ,medicine of this type of bone fracture in TCM; Describing the development and tenet of reduction manipulation and splint immobility; Suming up the development of the anatomy ,the reason of injury, the research of biological mechanism ,diagnosis and treatment. 48 examples were collected according to the reality. We divided these patients depending on the score of Lidstrom X—ray and Cooney wrist the into three groups. One group (16 examples) were treated by Splint immoblilty and group two(18 examples) were treated by external fixator, the third group(14 examples) in the worst situation were treated by both external fixator and Splint immoblilty .The three groups were observed for 8 weeks. The patient was evaluated by the Cooney wrist and the Lidstrom X—ray form.Result: Through clinical research, the result shows that difference is obvious .The group were treated by both external fixator and Splint immoblilty in the distal radial fracture more instability and X-ray performance were compared with bad situations to other 2 groups, restoreing the wrist dissection structure, working ability, the wrist forward sphere of action and the wrist grading aspect can achieve same effects with other 2 groups , and the replacement loss rate is also better than the group with pure Splint immoblilty. In restores the forward power of gripping aspect, the group with external fixator and Splint immoblilty is better than the group with pure Splint immoblilty.
