Objective:To explore the influences of vibration in vitro on human tonsillar mononuclear cells (TMCs) in IgA nephropathy patients and chronic tonsillitis patients, focus on the expression of BAFF and aberrantly glycosylated IgAl.Methods:In our study, we collected the cases taken tonsillectomy surgery in our hospital. There were14patients with IgA nephropathy (IgAN) and12with chronic tonsillitis (CT) and sleep apnea syndrome who don’t have renal diseases. Tonsillar mononuclear cells (TMCs) were isolated and cultured for24hours to make the cells stable, then the cells were divided into six test groups. Group A were collected as base value, group B were cultured for another72hours without vibration as control group. Group C-F were subjected to vibration for short time periods of1minute,3minutes,5minutes, and10minutes separately, and then got cultured for another72hours in the same conditions. Concentration of IgAl and IgAl aberrant O-glycosylation level in the culture supernatants were measured by specific enzyme-linked immunosorbent assay (ELISA). The gene expression of B-cell-activation factor (BAFF), core1(31,3-galactosyltransferase (C1GALT1), and core β1,3GalT- specific molecular chaperone (Cosmc) in tonsillar mononuclear cells were examined by quantificational real-time PCR. Observe the effects of vibration stimulation between different groups of patients, and on different time periods.Results:1. Before vibration stimulation was given, the base value of IgAl, IgAl aberrant O-glycosylation levels and BAFF mRNA levels were higher in IgAN patients in comparison with those in CT patients. Meanwhile, the gene expression of C1GALT1and Cosmc were lower in patients with IgAN.2. After vibration stimulation for different time periods, IgAl concentration, IgAl aberrant O-glycosylation levels and BAFF mRNA levels of TMCs in IgAN patients were significantly higher than those without vibration. Whereas the expression levels of C1GALT1and Cosmc were significantly lower than those without stimulation in IgAN patients. In CT patients, the expression of BAFF levels increased after vibration stimulation. While no significant differences of other factors was observed in CT patients.3. When vibrated for1minute, BAFF mRNA expression levels increased in both IgAN and CT patients, but the increasing degree was similar. The changes can lead to IgAl secretion increase in IgAN patients, but IgAl secretion didn’t change in CT patients. When vibrated for more than1minute, BAFF mRNA levels didn’t change in CT patients, while BAFF mRNA levels increased again in IgAN patients, and the increasing degrees were more than that of CT patients. Among the groups subjected to different time periods of vibration, there were no significant differences of C1GALT1and Cosmc gene expression in both groups.4. Pearson correlation analysis showed that, there were significant negative correlations between expression of C1GALT1&Cosmc and IgAl aberrant O-glycosylation level. There were significant positive correlations between BAFF mRNA levels and IgAl concentration.Conclusion:Vibration stimulation can activate BAFF expression in both IgAN and non-IgAN patients. But the influence may be stronger in IgAN patients. It can also induce IgAl secretion in patients with IgAN, and bring to IgAl aberrant O-glycosylation by suppressing C1GALT1and Cosmc expression. To some extent, the vibration on TMCs in vitro can imitate the physical stimulation the palatine tonsils suffered in the throat when speaking. The results confirm the impact of high intensity speaking on the TMCs. So we suggested that avoiding speaking loudly would be beneficial for patients with IgA nephropathy.
