| Objective: Stroke is a common disease with high incidence,high mortality and high morbidity,it has become a global healthy problem.With the change of the lifestyle and the arrival of the aging society,the incidence of stroke has increased year by year,and the onset age is gradually younger.The disability rate of stroke patients has not obviously improved under the constant development of the medical technology and treatment.About 70%-80% of stroke patients have different level of disability.In our country,most patients choose to go home for rehabilitation after drug therapy in acute phase.Due to the lack of knowledge about stroke,the poor compliance of discharge guidance,the out-of-hospital problems can’t be solved effectively,it leads to a bad prognosis of stroke.Health education after discharge can provide stroke patients with professional guidance and help to meet the health needs of them.This study was designed to implement the healthy education based on WeChat in stroke patients discharged,to explore whether it is possible to improve knowledge about stroke patients and treatment compliance,thereby improving activities of daily living(Activities of Daily Living,ADL)and satisfaction,so as to provide scientific basis for carrying out extended care in stroke patients in future.Methods: The study subjects were selected from May 2015-December2014 in the second hospital of Hebei Medical University,110 cases of stroke patients in the Department of Internal Medicine who have been discharged.Study samples were randomly divided into control group and intervention group.The two groups of patients were routinely given to health education during hospitalization.The control group after discharge was conducted conventional mode of care,the intervention group of patients after discharged was not only used conventional mode,but also applied extended careintervention based on WeChat healthy education.Set up a extended care team,establish stroke patient profiles.There was one person who was responsible for providing health knowledge by WeChat healthy education regularly and 8times follow-up.It was observed from two groups after 3 months of intervention,subjects in two groups were with or without statistically significant difference occurred in level of knowledge of stroke patients,the compliance of discharge guidance,the ability of daily living activities and patients’ satisfaction before and after the intervention.All data were analyzed by using SPSS17.0 statistical software.Results:1 Comparison of two groups of patients with general informationThe difference between the control group and intervention group was not statistically significant(P>0.05)in age,gender,educational level,marital status,occupation,medical expenses payment,income levels,complications and other general data.2 Comparison of knowledge of diseaseBefore the intervention,there was no significant difference between the two groups(P>0.05),have obvious comparability.The difference was not statistically significant in control group(P~*>0.05).However,after 3 months of intervention,the scores of knowledge of stroke in the intervention group were significantly increased compared with that before the intervention(P~*<0.05).And after the intervention,the score of the intervention group was significantly higher than that of the control group(P<0.05),and the difference was statistically significant.3 Comparison of treatment complianceComparing compliance of discharge nursing plan between the two groups in 3 months after discharge,four aspects including alcohol and tobacco control(P<0.05),rehabilitation training(P<0.05),regular medication(P<0.05),periodic review(P<0.05)in the intervention group was significantly higher than that of control group,and the difference is statistically significant.There were no significant differences on one aspect of balanced diet(P>0.05)4 Comparison of ADL scoresComparing the ADL score between the two groups of patients at discharge(P>0.05),the difference was not statistically significant,thus it has comparability.However after 3 months of intervention,compared to ADL scores at discharge hospital,the ADL scores in both groups were improved in varying degrees(P~*<0.05),and the intervention group had higher ADL scores compared with the control group(P<0.05).5 Comparison of discharge satisfactionIn three months after discharge,the score of nursing service satisfaction of patients in intervention group is(86.92±9.50)and that of the control group is(67.96±10.43),the difference of the two group is statistically significant(P<0.05).Conclusions:1 WeChat healthy education can improve the level of knowledge of stroke disease in stroke patients effectively.2 WeChat healthy education can improve the compliance of stroke patients in the control of tobacco,rehabilitation exercise,regular medication and periodic review effectively,but has no improvement in balanced diet.3 In three months after discharge,the ADL score of both the control group and the intervention group were significantly improved.Due to the implementation of WeChat healthy education,patients can get continuous rehabilitation and health guidance.Therefore,the increase of ADL score in the intervention group was more obvious than the control group.4 The study found that stroke patients’ satisfaction with nursing service is relatively low.WeChat healthy education may improve patients’ satisfaction obviously. |
