Clinical Study On The Effect Of Bushen Tiaogan On Dysfunctional Uterine Bleeding Of Different Ages

Part One Clinical observation of Bushen decoction on perimenopausal dysfunctional uterine bleeding with kidney deficiency and blood stasisObjective: By means of observation and comparison of the effect of the Bushen and Kun Lingwan on anastalsis, endometrial thickness, changes of hormone level, and recurrences rate of patients, that are diagnosed as perimenopausal dysfunctional uterine bleeding with kidney deficiency and blood stasis, this research aims to explore integrative medicine efficacy of Bushen, and to provide useful therapy and formula for clinical treatment of perimenopausal dysfunctional uterine bleeding.Methods: Selecting the gynecological outpatients 60 cases that are diagnosed as perimenopausal dysfunctional uterine bleeding with kidney deficiency and blood stasis in Hebei Chinese Medicine Hospital, Hebei University of Chinese Medicine and Shijiazhuang Le Ren Tang Pharmacy from September 2013 to April 2015. The 60 cases were randomly divided into two groups, treatment group and control group. Bleeding period: the treatment group was given BushenⅠand Ⅱ, the control group was given Kun Lingwan and Bushen Ⅱ. Stop bleeding: the treatment group was given BushenⅠand the control group was given Kun Lingwan. According to this method, three menstrual cycles are continuously taking medicine. After the end of treatment, observe the effect of hemostasis, clinical effect, Serum sex hormone level and Endometrial changes of the patients. We carried on pre-and post-treatment self-control and control between the two groups.Results: 1 Hemostatic situation: The vaginal bleeding of the two groups were improved in different degree. After taking the medicine, the control group stopped bleeding for an average 6.63±2.65 days, the treatment group was 4.30±1.73 days; 2 Endometrial changes: The endometrium of the two groups patients were thinner after treatment. After treatment, the two group were significant differences from those before treatment. After treatment compared with the control group, the treatment group of patients with thin endometrium and there was significant difference（P< 0.05）; 3 Comparison the changes of sex hormone level: Two groups of patients before treatment of follicle-stimulating hormone（FSH）, luteinizing hormone（LH）, estradiol（E₂）, Progesterone（P） value had no significant difference（P> 0.05）, can be comparabled. The serum levels of E₂, LH and FSH were decreased significantly. There were significant differences in the control group before and after treatment（P< 0.05）, the treatment group was the same as the control group. Progesterone in the control group was decreased after treatment, but no statistical significance; After treatment, the treatment group compared with the control group, FSH, LH, E₂, P has significant differences and there was statistical significance（P< 0.05）; 4 Comparison the effect of Clinical curative: the total effective of control group is 76.6% and 93.3% in treatment group; 5 Recurrence rate comparison: After half a year, the recurrent rate of control group was 32.0%, and the treatment group was 10.3%.Conclusion:1 Bushen decoction and Kunling wan on kidney deficiency and blood stasis perimenopausal dysfunctional uterine bleeding plays a role in the treatment. Bushen decoction has a good effect to promote endometrial exfoliation and repair.2 Bushen decoction and Kunling wan have a regulatory effect on sex hormones to patients that are diagnosed with perimenopausal dysfunctional uterine bleeding of kidney deficiency and blood stasis.Bushen decoction can significantly reduce luteinizing hormone, follicle-stimulating hormone, estradiol, progesterone, prompting sex hormone levels in more coordinated.3 Bushen decoction has good curative effect on the patients who are perimenopausal dysfunctional uterine bleeding of kidney deficiency and blood stasis.The effect of Bushen decoction is related to the regulation of endometrium and sex hormone levels.4 Side effects and adverse reactions of Bushen decoction is relatively small and Bushen decoction for the treament of perimenopausal dysfunctional uterine bleeding has the characteristics of stability and long-term effect and the low rate of recurrence. Part Two Clinical observation of Tiaogan decoction on child-bearing period dysfunctional uterine bleeding with liver and kidney deficiency syndromeObjective: In this study, by observing the effect of Tiaogan decoction and Xiao Yao pill+Kun Ling wan on hemostasis effect, endometrial thickness, changes of hormone level, and recurrences rate of patients, that are diagnosed as adult dysfunctional uterine bleeding with stagnation of liver and kidney deficiency. This research aims to explore integrative medicine efficacy of tiaogan, and to provide useful therapy and formula for clinical treatment of child-bearing period dysfunctional uterine bleeding.Methods: Selecting 60 cases who are diagnosed as dysfunctional uterine bleeding patients with stagnation of liver and kidney deficiency, at the age of 23⁴2, in Hebei Province Hospital of traditional Chinese medicine medical hall, Guo Yi Tang of Hebei University of Chinese Medicine and Shijiazhuang lerentang Museum of traditional Chinese Medicine from September 2013 to June 2015. The 60 cases were randomly divided into two groups: treatment group and control group. Bleeding period: the treatment group was given TiaoganⅠand Ⅱ, the control group was given Xiao Yao pill, Kun Ling wan and Tiaogan Ⅱ. Stop bleeding: the treatment group was given TiaoganⅠand the control group was given Xiao Yao pill and Kun Ling wan. According to this method, three menstrual cycles are continuously taking medicine. After treatment compared with the control group,the treatment group of patients with thin endometrium and there was significant difference（P< 0.05）; 3 Comparison the changes of sex hormone level: The two groups were compared before and after treatment with LH and E₂ values were significantly decreased, there was significant differences in the two groups（P<0.05）. FSH and P compared with that before treatment was increased; After treatment two groups was compared, LH and E₂ in the treatment group were decreased and the difference was significant（P<0.05） and the ratio of estradiol and progesterone was reduced; 4 Comparison the effect of Clinical curative: the total effective of control group is 63.3% and 86.7% in treatment group; 5 Recurrence rate comparison: After half a year, the recurrence rate in the control group is 37.9%, treatment group is 11.1%.Conclusion:1 Tiaogan decoction and Xiao Yao pills+Kunling wan plays a role in the treatment on childbearing age of dysfunctional uterine bleeding with liver and kidney deficiency. Tiaogan decoction has more obvious effect on endometria.2 Tiaogan decoction and Xiao Yao pill+Kunling wan have a regulatory effect on sex hormones to patients that are diagnosed with childbearing age dysfunctional uterine bleeding with liver and kidney deficiency. The role of Tiaogan decoction has more prominent, it can reduce LH and E₂, increase FSH and P, and decrease the ratio of estrogen and progesterone.3 Therapeutic effect of tiaogan decoction to childbearing age dysfunctional uterine bleeding with liver and kidney deficiency is related to the regulation of endometria and sex hormone.4 Side effects and adverse reactions of Tiaogan decoction is relatively low and Tiaogan decoction for the treatment of childbearing age of liver and kidney deficiency of dysfunctional uterine bleeding has the characteristics of stability and long-term effect and the low rate of recurrence.
