| Objective: Diabetic nephropathy is one of the most typical microvascular complications of diabetes,and it is one of the leading causes of chronic renal failure and diabetes mortality.The current clinical treatment of the disease mainly adopts low protein diet,control blood sugar,reduce blood pressure,blood fat,application of Renin-angiotensin system blockers and renal replacement therapy,etc.While short-term effect is obvious,but it is hard to prevent the occurrence of diabetic nephropathy development.Looking for reliable efficacy,less side effects of drugs,has become a hot spot in clinical research.Research shows that deficiency of Qi and Yi,deficiency of spleen and kidney is the basic pathogenesis of this disease,phlegm and blood stasis are the important pathological products of this disease,which are always the main pathology of diabetic nephropathy.Supplementing qi and nourishing yin and kidney tonic,hlegm-resolving and collateral-unblocking is the main treatment of the disease.Supplementing Qi and Nourishing Yin,Invigorate the kidney and stop emission,Resolving phlegm and dredging collaterals are the main treatment of the disease.Bushenhuazhuo capsule has the functions of nourishing liver and kidney,benefiting qi,invigorating spleen,resolving phlegm and dredging collaterals.Fermented Cordyceps preparations(Bailing capsule)has the functions of invigorating lung and kidney essence and qi,two drugs are widely used in the treatment of diabetic nephropathy,but both use less reported.In order to further observe the combination effect,this study aims to through the western medicine based on conventional treatment combined with kidney turbidity capsule and preparations made from cordyceps sinensis,and respectively by kidney turbidity capsule and preparations made from cordyceps sinensis as contrast,provide the reference for the diabetic nephropathy in clinical and scientific research of TCM.Methods: A total of 90 cases of III and IV in patients with diabetic nephropathy in the south of Cangzhou Central Hospital of Hebei province from March 2014 to August 2015 were collected.90 cases of patients with completely randomized design into the Bushenhuazhuo capsule group(Bushenhuazhuo group),Bailing group,Bushenhuazhuo capsule combined with Bailing capsule(combining treatment group),Study period for 3 months,to compare three groups of traditional Chinese medicine clinical symptoms,blood sugar,glycosylated hemoglobin,urine trace albumin and renal function,24 h urine protein quantitative changes before and after treatment and clinical efficacy and safety.Results:1 Three groups after treatment 1 month,2 months and 3 months,the fasting plasma glucose,postprandial 2 hours blood sugar,glycosylated hemoglobin were decreased obviously than before,the difference was statistically significant(P<0.05 or P<0.01);after 3 months of treatment,combined treatment group in reducing fasting blood glucose,postprandial blood glucose two hours,the glycosylated blood red egg was better than Bushenhuazhuo group and Bailing Capsule group,the difference was statistically significant(P<0.05).2 Three groups after treatment 1 month,2 months and 3 months,Urinary albumin,24 hours urinary protein,blood urea nitrogen,serum creatinine were decreased than before treatment,the glomerular filtration rate was significantly higher than before treatment,the difference was statistically significant(P<0.05 or P<0.01);after 3 months of treatment,combined treatment group in reducing urine trace albumin,a 24-hour urinary protein,blood urea nitrogen,serum creatinine,increaseing kidney glomerular filtration rate was better than Bushenhuazhuo group and Bailing Capsule group,the difference was statistically significant(P <0.05).3 Three groups after treatment 1 month,2 months and 3 months,TCM syndrome score was significantly lower than before treatment,the difference was statistically significant(P<0.05 or P<0.01);after 3 months of treatment,combined treatment group in reducing TCM syndrome score was better than Bushenhuazhuo group and Bailing Capsule group,the difference was statistically significant(P<0.05).4 After 1 month treatment,Bushenhuazhuo group,Bailing Capsule group and combination group curative effect were 71.43%,68.97%,81.81%,three groups of patients with clinical total curative effect is had no statistical difference(P>0.05),But the effect of combined treatment group was better than that of the kidney and the kidney and the capsule group.After 2,3 months of treatment,the effect of Bushenhuazhuo group,Bailing capsule group and combined treatment group curative effect were 75% and 72.61%,87.88% and 82.14%,79.31%,90.91%,Bailing capsule group compared with the combined treatment group,there are statistically significant(P<0.05),Bushenhuazhuo group compared with the Bailing capsule group,there was no statistical significance(P>0.05).Conclusion: On the basis of conventional western medicine therapy,Bushenhuazhuo capsule combined with Bailing capsule group,which could significantly improve the clinical symptoms of Chinese medicine,protect renal function,reduce urinary protein and other related indicators of kidney damage,delay the development of glomerular sclerosis,improve diabetic nephropathy patients with glucose metabolism disorders,the curative effect was better than that of Bushenhuazhuo group and Bailing capsule group. |
