| Objective:To investigate the risk management and the effects of the joint application of Hazard Vulnerability Analysis(HVA) and Failure Mode and Effect Analysis(FMEA)in the Central Sterile Supply Department(CSSD) in a tertiary hospital; To improve the efficiency and effectiveness of risk management, reduce risk and ensure hospital service quality and safety; To provide methods and references for risk assessment and improvement and data references for the CSSDs.Methods:1. Research objectThis research project has taken the CSSD in a tertiary hospital in Ganzhou city,Jiangxi province as its research object. Data’s resources before and after the risk prevention measures implementing were respectively taken as the control group(2014) and the experimental group(2015).2. Methods and outcome measuresHVA was done to identify high risk events in the CSSD by a risk control team.FMEA was done to screen the key failure modes and potential risks of high risk events, and preventive measures were developed and implemented based on the result of the analysis. Before and after the measures, the Hazard Specific Relative Risk(RISK) value and the Risk Priority Number(RPN) value of the failure modes of the top five events with the highest RISK value were given specific analysis.3. Statistical analysisAll the data was double checked, and a computer database was established based on them so as to be analyzed by the software of SPSS 21.0. The RISK value of the risk events and the RPN value of the failure modes of the risk events were described as x ± s. The top five RISK values of the risk events and their RPN value of the five key failure modes were analyzed by t- test, and the result(P<0.05) suggests that it has statistical significance.Results:1. The automatic cleaning and disinfecting machine failure, low-temperature plasma sterilization machine failure, sharps injuries, pulsation vacuum pressure steam sterilizer failure, and steam leakage, among the possible 31 risk events by HVA, were identified as the top five high risk events in the CSSD.2. Both the RISK values of the five high risk events(P<0.05)and their RPN values of the key failure modes(P<0.05)were statistically decreased by FMEA and improving measures.Conclusion:The joint application of HVA and FMEA in the CSSD of a hospital can be of great significance in identifying the key points of management risks, improving the efficiency and effectiveness of risk management, and reducing the risk vales(RISK value and RPN value), so as to ensure the hospital service quality and safety, and to provide some enlightenment on risk assessment methods and data references of RISK and RPN values for the CSSDs. Therefore, it is worth promoting. |
