| Objective: To collect the clinical data of 111 elderly diabetes inpatients receiving geriatric assessment.To compare of the levels of blood glucose,blood lipid,blood pressure,Senile syndrome components and other factors between cognitive normal group and cognitive impairment group,in order to discuss the relationship of cognitive impairment in elderly with diabetes.Methods: To collect the data of 111 elderly diabetes inpatients of Geriatric Department of the Second Affiliated Hospital of Dalian Medical University from November 2014 to November 2015.According to the results of mini mental state scale(MMSE),all the patients were divided into the cognitive normal group(CN)and cognitive impairment group(CI).To compare the two groups,including gender,age,education level,living conditions,marital status,systolic blood pressure,diastolic blood pressure,BMI,WHR,FPG,FINS,C-P,C-P,2hPG,2hINS,2hC-P,HOMA-IR,HbA1 c,TC,TG,HDL-C,LDL-C,ALB,prealbumin,mini nutritional assessment short form(MNA-SF)score,ability of daily life(ADL)score,geriatric depression scale(GDS)score,the international consultation on incontinence questionnaire short form(ICI-Q-SF)score,whether with coexistence of sick,multiple drugs,diminution of vision,urinary incontinence,constipation,chronic pain,hearing loss.To compare measurement data between two groups with two independent sample t test,to compare of the rate with chi-square test,finally,statistical software was used to analyze the multiple factors logistic regression analysis to explore the risk factors of cognitive impairment in the elderly diabetes patients.Results: 1.In the 111 elderly patients with diabetes,The incidence of coexistence ofsick,multiple drugs,diminution of vision,urinary incontinence,cognitive impairment,constipation,chronic pain,ability of daily life decline,depressed mood,malnutrition,hearing loss was 99.09%,90.09%,82.88%,63.96%,62.16%,44.14%,37.84%,36.94%,31.53%,25.52%,13.51% respectively。2.In the 111 elderly patients with diabetes.The average systolic blood pressure was(140.51±21.97)mmHg in CN group,and(148.41±21.13)mmHg in the CI group.The average diastolic blood pressure was(77.32±11.68)mmHg in CN group,and(81.89±11.21)mmHg in the CI group.Each factor was significantly higher in CI group.3.In the 111 elderly patients with diabetes.There were 11 patients in CN group with coronary heart disease,and 13 patients in CI group.The incidence of coronary heart disease(15.71%vs31.72%)were significantly higher in CI group than that in CN group(P≤0.05).4.In the 111 elderly patients with diabetes.The average score of ADL was(16.52±4.43)in CN group,and(19.22±656)in CI group.The average score of GDS was(3.13±2.61)in CN group,and(4.61±3.01)in CI group.Each factor was significantly higher in CI group.5.To use logistic regression analysis,diastolic blood pressure(beta=1.059,95%CI 1.017-1.103),ADL score(beta=1.084,95%CI 1.008-1.167)and GDS score(beta=1.189,95%CI 1.016-1.391)were associated with cognitive impairment(P<0.05).Conclusion: 1.Geriatric syndrome is common in the elderly patients with diabetes,such as coexistence of sick,multiple drugs,diminution of vision,urinary incontinence,ability of daily life decline,constipation,chronic pain,cognitive impairment,depressed mood,malnutrition,hearing loss.2.The average systolic blood pressure and diastolic blood pressure are higher in cognitive impairment group.3.Cognitive impairment which are more likely to happen in elderly diabetes patients with coronary heart disease.4.The average of ability of daily life score,geriatric depression scale score are higher in cognitive impairment group.5.Diastolic blood pressure,ability of daily life declining,depression mood maybe independent risk factors of cognitive impairment in elderly diabetes patients. |
