[Objective] To observe the effect of oral plus enema administration of traditional Chinese medicine(TCM) on postoperative recurrence of endometriosis(EMT) and underlying mechanisms, which illuminates further etiologies and offers novel therapeutic approaches.[Method] 60 cases of patients diagnosed with EMT are randomly divided into herbgroup(n=30) and gestrinonegroup(n=30). Postoperative observation is performed on patients concerning TCM-related signs of efficacy, symptoms score,dysmenorrhea and dyspareunia score, serum CA125 level, recurrence ratio, pregnancy success as well as side effects. Statistic differences are determined by analytic methods.[Results]1.signs of efficacy and symptoms score: symptoms score is almost the same before treatments, which is significantly attenuated by either herb or gestrinone administration. The effectiveness rate of herb group is 96.67% while that of gestrinone group is 66.67%. Statistic analysis shows remarkable difference in effectiveness between two groups(p<0.05), which means herb treatment is better than gestrinone.2.dysmenorrhea and dyspareunia score: no difference in dysmenorrhea and dyspareunia score of two groups shows before any postoperative treatments, which is dramatically alleviated with either treatment(p<0.05). herb group shows better therapeutic effect compared to gestrinone group(p<0.05).3. CA125level: herb and gestrinone have comparable effect on EMT(P>0.05).CA125 return into normal level at the end of treatments with continuously descending trend.4.Recurrence rate: the recurrence rate is 0% with either a course of TCM or gestrinonewhile it goes to 6.67% and 9.99%, respectively. No significance is showedbetween two groups(P>0.05) which means two treatments are effective and comparable in terms of post-operational recurrence.5.Pregnancy success: in patients diagnosed by EMT complicated with infertility, 8patients are confirmed with pregnancy 3 months after TCM treatment(8/16, 50%)while 4 patient is pregnant(4/15, 26.67%, P<0.05) from gestrinone group. 12 patients are confirmed with pregnancy 2 years after TCM treatment(12/16, 75%) while 8patients are pregnant from gestrinone group(8/15, 53.33%, P<0.05). it concludes that TCM is better than gestrinone concerning pregnancy success. This better protective effect is achieved by the fact that estrinone leads to amenorrhea causing postponed pregnancy.6.Side effect: a patient from TCM treatment group shows side effect of gastrointestinal discomfort. 21 patients show side effects such as liver injury,gastrointestinal discomfort, facial acne as well as dramatic gain of weight(P<0.01). It concludes that TCM is more safe with less side effects.[Conclusion]Compared to gestrinone treatment, TCM group shows similar therapeutic effect in terms of postoperational recurrence and CA125 while TCM treatment shows better protection in aspect of pregnancy success, dysmenorrhea, dyspareunia, signs of efficacy and symptoms score. Besides, TCM shows less side effects as well as no amenorrhea. It can be concluded that oral plus enema administration of traditional Chinese medicine can effectively prevent EMT postoperational recurrence, attenuate dysmenorrhea and increase the pregnancy success with less off-target effects. The novel approach shows a promising clinical application.
