Objective:To observe the clinical therapy effect of lyophilized recombinant human brain natriuretic peptide(rh-BNP)for mechanical ventilation patients complicated with heart failure and respiratory failure.Methods:53 patients of mechanical ventilation complicated with heart and respiratory failure were collected and divided into the rh-BNP combined with the routine treatment group(treatment group,26cases)and the routine treatment group(control group,27cases)according to use rh-BNP or not.The two groups were given conventional treatment such as inotropic,diuretic and vasodilator.The rh-BNP group on the basis of conventional therapy with rh-BNP added.In all the patients,heart rate and systolic blood pressure and the BNP level were assessed before and at 24h,48h,72h and 1w after drug administration,and the urine output along with hemodynamic parameters was recorded after drug administration.The outcome and withdrawal machine survival rate after28 days also need to be assessed.The patients in rh-BNP group were given rh-BNP at the initial dose of 1.5μg/kg by with an intravenous bolus injection followed by infusion at the rate of 0.015μg·kg-1·min-1 for 48 to72h.Results:The total effective rate of treatment group was higher than that of the conventional treatment group(80.77% vs 62.96%).The mechanical ventilation time and ICU hospitalization time of treatment group significantly lower than the control group,with statistical significance(P < 0.05).Conclusion:The rh-BNP can improve symptoms and heart function,reduced the ventilation and hospitalization time of mechanical ventilation patients complicated with heart failure and respiratory failure.
